Merck/Moderna's Phase 3 INTerpath-001 topline (N=1,137) of intismeran autogene plus pembrolizumab in resected stage IIB-IV melanoma dominated the week, alongside a JITC correlative analysis of TILT-123 plus IL-2-free TIL therapy, continued debate over the RP1/nivolumab approval, and BioNTech's futility stop in colorectal mRNA vaccine.

Immune-cytokine signature predicts survival in patients with advanced melanoma treated with oncolytic adenovirus TILT-123 and chemotherapy and IL-2-free adoptive TIL therapy. โ Haybout Lyna et al., J Immunother Cancer
17 patients with checkpoint-inhibitor-resistant metastatic melanoma received up to six intratumoral TILT-123 injections followed by TIL infusion, without lymphodepleting chemotherapy or IL-2 post-conditioning. Tumor biopsy analyses revealed early innate immune activation marked by natural killer-cell expansion and cytotoxic gene upregulation, followed by increased intratumoral T-cell infiltration; enrichment of CD27+CD28+ memory-precursor CD8+ T cells was associated with favorable clinical outcomes, while elevated monocytic myeloid-derived suppressor cells and angiogenic/inflammatory cytokines after combination treatment were associated with disease progression. Correlative analysis in pooled TILT-123 cohorts identified serum epidermal growth factor as a candidate biomarker for stratifying and monitoring patients.
Clinical bottom line: With survival follow-up extended to March 2026, the TUNINTIL correlative analysis pins outcomes of this chemotherapy- and IL-2-free TIL approach to CD27+CD28+ memory-precursor CD8+ T cells and to myeloid suppressor/cytokine burden, and nominates serum EGF as a stratification biomarker for the next round of trials.

Phase 3 INTerpath-001 (N=1,137) met its primary recurrence-free survival and key secondary distant metastasis-free survival endpoints, demonstrating statistically significant and clinically meaningful improvements compared with pembrolizumab alone in completely resected stage IIBโIV melanoma โ marking what clinicians described as the first positive Phase 3 trial of an individualized neoantigen therapy and of an mRNA-based cancer therapy. Intismeran autogene, in which up to 34 patient-specific neoantigens can be selected, was framed by Moderna as the first and only combination regimen to demonstrate a clinically meaningful improvement over pembrolizumab alone, a current standard of care, and by Melanoma Institute Australia's principal investigator as the first Phase 3 study to show intismeran plus pembrolizumab can reduce the risk of recurrence or death versus pembrolizumab alone, with full results to be presented at a congress later this year. Commentators simultaneously described the readout as a landmark moment in melanoma, cancer immunology, and IO drug development, characterized it as derisking cancer vaccines in Phase 3 melanoma, and noted it as the first large randomized controlled trial of a cancer treatment vaccine. With only topline RFS and DMFS announced, clinicians outside melanoma immediately asked whether adjuvant personalized vaccine trials fully accrued or ongoing in lung โ and other solid tumors โ can reproduce the result, while the central debate remains how adjuvant melanoma practice should respond before magnitude of benefit, durability, and overall survival are known.

BioNTech's decision to stop its colorectal cancer trial for futility, occurring alongside Moderna's positive INTerpath-001 melanoma readout, prompted clinicians to articulate a basic principle: evidence belongs to the population studied, with different cancer, setting, patients, and result separating the two programs. One commentator who had described personalized cancer vaccines as having crossed a major line with positive Phase 3 INTerpath-001 in resected melanoma also noted participation in the adjuvant RCC trial and anticipation of its results, while melanoma specialists argued that much can be built on the mRNA platform to improve the outcomes of patients with cancer. The central question is whether BioNTech's colorectal futility stop should temper cross-tumor extrapolation from the melanoma neoantigen readout, or whether tumor immunogenicity is the deciding variable.

Phase 3 INTerpath-001 is positive: the individualized mRNA neoantigen therapy intismeran (V940/mRNA-4157) + pembrolizumab significantly improved both RFS/DMFS vs pembrolizumab alone after resection of high-risk melanoma.
For years, the idea of designing a treatment around the unique mutations of each patientโs tumor seemed aspirational. Now we have the first positive Phase 3 trial (N=1137) of an individualized neoantigen therapyโand of an mRNA-based cancer therapy.
Personalized cancer vaccines are moving from promise to reality.
Now we need to see the magnitude of benefit, durability, and ultimately OS. But today is an important day for the fieldโand hopefully for our patients. ๐

INTerpath-001 clinical trial represent a landmark moment for adjuvant melanoma treatment.
Melanoma Institute Australia participated in this trial, with Professor Georgina Long AO as Principal Investigator. During this trial, patients with resected stage IIB-IV melanoma were treated with intismeran, a personalised mRNA-based therapy designed around the unique profile of a patientโs own tumour, given with pembrolizumab versus pembrolizumab alone.
ย ย
โThis is the first Phase 3 study to show that intismeran, a treatment based on the unique mutational โfingerprintโ of a patientโs own tumour, given in combination with pembrolizumab can reduce the risk of recurrence or death in patients with completely resected stage IIB-IV melanoma compared to pembrolizumab alone. Intismeran in combination with pembrolizumab has the potential to establish a new treatment paradigm in the adjuvant melanoma setting, helping patients remain cancer-free for longer.โ - Professor Georgina Long AO, Medical Director, Melanoma Institute Australia
