๐Ÿ“ฌ Melanoma Intelligence Digest โ€” Week 2026-W40

A JAMA review on cancer vaccines drove the week's melanoma conversation alongside spatial immune-triad biomarkers of checkpoint response, a German cohort linking false-negative melanoma diagnosis to 8.2 years of life lost, CDKN2A variant-specific risk, and a preview of full Phase 3 INTerpath-001 data at ESMO.

JAMA review positions cancer vaccines as moving from concept to clinic, with melanoma among the approved uses

A JAMA Review describes cancer vaccines being evaluated for treatment and prevention across multiple malignant neoplasms as advances in tumor immunology, antigen discovery, and vaccine delivery reshape the field; current approved uses span melanoma, prostate cancer, and bladder cancer, with vaccines stimulating adaptive immunity against neoantigens, nonmutated tumor proteins, or tumor-derived antigens. Melanoma-focused clinicians characterized vaccines as imminent for the disease, framing melanoma as the lead indication with other cancers expected to follow. The open question for clinicians is which resected or advanced melanoma patients should be prioritized for vaccine-based strategies outside a trial, given the approved uses documented so far.

Spatial immune triads, not T-cell abundance, separate checkpoint-responsive from resistant melanoma

In an analysis of 32 lymph-node metastases โ€” including only six ICI responders โ€” the distinguishing feature of responding tumors was spatial organization: clonally expanded CD8โบ tumor-resident T cells, CD4โบ resident T cells, and DC3 dendritic cells assembled into local immune triads found only in responding tumors, with TCF1โบ progenitor-like resident CD8โบ cells also enriched, suggesting these niches may continuously replenish cytotoxic effectors. Resistant tumors still carried strong IFN-ฮณ programs, but alongside exhaustion and an immunosuppressive macrophage state, demonstrating that an inflamed tumor is not necessarily an effective one. The analysis is associative and cannot establish that these triads cause response, leaving open whether spatial immune architecture is ready to underpin a prospective checkpoint-response biomarker assay in melanoma or whether a six-responder dataset is too thin to build on.

CDKN2A I49T carriers show far less melanoma than classic pathogenic variants but earlier-onset pancreatic cancer

In a Mexican cohort of 203 patients, CDKN2A I49T carriers had markedly less melanoma than carriers of known pathogenic or likely pathogenic CDKN2A variants (OR 0.089; P<0.001), while pancreatic cancer frequency was not significantly different between groups (OR 0.45; P=0.17). Pancreatic cancer did, however, appear at a younger age in I49T carriers โ€” 56 versus 71 years (P=0.026) โ€” a finding that complicates straightforward de-escalation of surveillance. The debate for specialists is whether CDKN2A I49T carriers should be counselled and surveilled for melanoma differently from classic pathogenic variant carriers, while maintaining pancreatic surveillance given the earlier-onset signal.

Update: full Phase 3 INTerpath-001 data set for ESMO on 24 October, with magnitude of benefit still unreported

Full Phase III INTerpath-001 results for intismeran autogene plus pembrolizumab โ€” an individualized neoantigen therapy designed with a deterministic machine learning bioinformatic algorithm โ€” are scheduled for presentation at ESMO on 24 October in patients with completely resected stage IIBโ€“IV melanoma; Merck and Moderna reported in August that the study met its recurrence-free and distant metastasis-free survival endpoints, with the combination also reported to have improved RFS and DMFS in resected melanoma. The October data are expected to answer how much benefit the combination provides relative to its side effects and the demands of manufacturing a treatment for each individual patient. Commentators have raised scalability โ€” a cancer vaccine built for one person โ€” as the central open question, and the practical debate is whether adjuvant individualized neoantigen therapy is worth that manufacturing burden for stage IIBโ€“III resected melanoma before the effect size is known, or whether it should wait for stage IV-resected patients.

RT @AponiaAnalytics: T-cell numbers may matter less than their neighborhood.

ICI-responsive melanoma was not simply richer in activated lymphocytes. The distinguishing feature was spatial organization: clonally expanded CD8โบ tumor-resident T cells, CD4โบ resident T cells and DC3 dendritic cells assembled into local immune triads found only in responding tumors.

TCF1โบ progenitor-like resident CD8โบ cells were also enriched, suggesting these niches may continuously replenish cytotoxic effectors rather than merely host terminally activated cells.

The counterpoint is equally interesting. Resistant tumors still carried strong IFN-ฮณ programs, but alongside exhaustion and an immunosuppressive macrophage state.

So an โ€œinflamedโ€ tumor is not necessarily an effective one. The relevant variable may be whether inflammation is organized into a functional local circuit connecting dendritic cells, helper T cells and renewable cytotoxic T cells.

The study is associativeโ€”32 lymph-node metastases included only six ICI respondersโ€”and cannot establish that these triads cause response.

But it argues that spatial immune architecture may be a more informative biomarker than T-cell abundance or IFN-ฮณ signaling alone.

RT @ACIR_org: Qing and Ghorani et al. explored mechanisms of secondary resistance (2ยฐR) to therapy (Treg depletion via anti-CD25 mAb + GM-CSF-expressing tumor cell vaccine) in B16 tumor- bearing mice. At 2ยฐR, the TME had reductions in activated, cycling, and effector T cells compared to at initial response, and the activated T cells showed evidence of clonal expansion, tumor-specific TCR engagement, and Fas/death receptor (DR) expression. Adding anti-FasL + repeated anti-CD25 dosing to the therapeutic regimen extended survival. In patients receiving ICB, T cell activation also correlated with DR expression, and these T cells became lost over time.

๐Ÿฆ‰Personalized mRNA therapy in melanoma: now we need the numbers

Full Phase III INTerpath-001 results for intismeran autogene plus pembrolizumab are scheduled for presentation at ESMO on 24 October. The trial enrolled patients with completely resected stage IIBโ€“IV melanoma.

Merck and Moderna said in August that the study met its recurrence-free and distant metastasis-free survival endpoints. The October data should answer a more practical question: how much benefit does the combination provide, and how does that compare with its side effects and the demands of making a treatment for each patient?

Those details will shape the role of individualized neoantigen therapy after surgery.
