Personalized mRNA neoantigen therapy dominates melanoma discussion as the BioNTech colorectal futility stop is used to bound INTerpath-001 to melanoma, while COMBI-I long-term OS, PIVOTAL daromun, SECOMBIT thymidine kinase, lifileucel outpatient toxicity, the COWBOY trial and imneskibart Phase 2 data all land, and a surrogate-endpoint meta-analysis of 1865 first-line patients tests ORR, PFS and TNTD against OS.

Evaluating the potential of candidate surrogate endpoints for overall survival in treatment-naive metastatic melanoma treated with anti-PD-1-based regimens with or without anti-CTLA-4 therapy. β Squifflet Pierre et al., J Immunother Cancer
Individual-patient data from 1865 patients enrolled in four randomized controlled trials of single-agent nivolumab or nivolumab plus ipilimumab were used to evaluate ORR, PFS, and time to next treatment or death (TNTD) as candidate surrogates for OS. For ORR, the individual-level OR of survival was 12.29 (95% CI 9.78 to 14.80) and the trial-level RΒ² was 0.62 (95% CI 0 to 1.00); for PFS, the individual-level Ο was 0.72 (95% CI 0.70 to 0.73) with trial-level RΒ² 0.73 (95% CI 0.27 to 1.00); and for TNTD, the individual-level Ο was 0.77 (95% CI 0.76 to 0.78) with trial-level RΒ² 0.77 (95% CI 0.37 to 1.00). At the trial level, the key requirement for surrogate validation, all three candidate endpoints demonstrated only moderate predictive ability for OS, while ORR exhibited a strong individual-level correlation with OS.
Clinical bottom line: In first-line metastatic melanoma treated with nivolumab-based regimens, ORR tracks OS tightly for the individual patient but none of ORR, PFS, or TNTD reaches more than moderate trial-level predictive ability, so registration decisions built on these endpoints still carry real uncertainty about survival.

How these are chosen: filtered for clinical relevance, grouped by the new development discussed, then ordered by engagement (bookmarks and reposts).
Phase 2 data presented at ASCO26 for imneskibart combined with low-dose subcutaneous aldesleukin and nivolumab in metastatic melanoma progressing on frontline doublet checkpoint inhibition showed a 33% objective response rate and 67% disease control rate in a setting where options narrow quickly. For context in the same post-anti-PD-1 refractory space, vusolimogene plus nivolumab was approved on an ORR of 24.2% with a median duration of response of 14.1 months. The open question is how these two approaches should be positioned relative to each other, and which patients are best suited to an IL-2-based combination versus the newly approved oncolytic option.

BRAF inhibition increases IGF1 signaling, and the IGF1βIGF1R axis shapes tumor-associated macrophage polarization and ECM organization; IGF1R-deficient macrophages render melanoma cells more sensitive to PLX4032, and macrophage IGF1R knockdown reduces tumor growth in vivo, placing the tumor microenvironment at the center of this resistance mechanism. A separate report shows that combining lymphodepleting chemotherapy with dual immune checkpoint blockade enhances clonal expansion of effector CD8+ TILs, delays tumor growth, and improves survival. Together these data press the question of whether the tumor microenvironment, rather than the melanoma cell itself, is where the next generation of BRAF-inhibitor resistance strategies should be directed.

The COWBOY trial examined induction vemurafenib plus cobimetinib prior to ipilimumab-nivolumab in patients with BRAF V600E/K-mutated melanoma and elevated LDH versus upfront immunotherapy, but was terminated prematurely due to slow patient accrual. Analysis of the over 70 randomized patients indicates more favorable outcomes in the upfront immunotherapy group. Whether an underpowered, prematurely closed randomized trial is sufficient to retire targeted-therapy induction ahead of ipilimumab-nivolumab in this high-LDH BRAF-mutant population remains contested.

Published in JCO Oncology Practice, the analysis titled "Outpatient Treatment-Related Toxicity After Hospital Discharge Among Patients Receiving Lifileucel for Advanced Melanoma" reports that incidence of new grade 3 or higher treatment-related adverse events and readmissions is low in patients with melanoma discharged after tumor-infiltrating lymphocyte therapy with lifileucel. These post-discharge safety data bear directly on whether more of the lifileucel recovery period can be delivered outside a TIL-experienced inpatient center, a question with practical access and resource implications for programs considering adoption.

RT @tompowles1: After the positive press release for the Moderna personalised cancer vaccine (adjuvant therapy in combo with pembro in melanoma) we get a negative press release (βimbalances in OSβ β‘οΈearly termination) for the BioNTech PCV (monotherapy, sequenced after chemo, in ctDNA+ve adjuvant colon cancer). Speculation is rife, but pretty risky without seeing the data. The trials have different combos, different cancer types, different endpoints etc. However, great melanoma data has previously struggled to translate into other βcoldβ cancers before. We need to see data for both these trials before making wider predictions for the future of PCVs.
