📬 GYN Oncology Intelligence Digest — Week 2026-W43

A phase 1 personalized neoantigen vaccine trial in high-grade ovarian cancer drew the week's largest audience, while ATALANTE's final OS, KEYLYNK-001's PFS-without-OS result, TUB-040's biomarker-unrestricted activity, and a PP2A molecular glue for PPP2R1A-mutant endometrial cancer defined the rest of the conversation.

Emerging Therapeutic targets and Early-phase drug Development in Gynecological Malignancies. — Ouali K et al., Ann Oncol

The review reports early clinical activity for agents against the DNA damage response targets WEE1, PKMYT1 and WRN, and against vulnerabilities linked to CCNE1 amplification and TP53 Y220C alterations. Antibody-drug conjugates have become a major therapeutic class. Immunotherapy now extends beyond checkpoint inhibitors to bispecific antibodies, T-cell engagers, engineered T-cell receptor therapies and CAR-T cells. Tumor heterogeneity, biomarker complexity and acquired resistance remain the main barriers.

Clinical bottom line: Clinicians building molecular-profiling-driven trial referral pathways in recurrent gynecologic cancer can cite this review, but it changes no current treatment decision.

Personalized neoantigen-targeting cancer vaccines in ovarian cancer: a phase 1 trial. — Sellars MacLean C et al., Nat Med

NeoVax combines synthetic long peptides against patient-specific neoantigens with poly-ICLC and delayed anti-PD-1. It was tested in ten patients with platinum-sensitive ovarian cancer after platinum doublet chemotherapy. Nine of 12 evaluable Cohort B patients were vaccinated (75%, 90% CI 47-93%), compared with 1 of 9 Cohort A patients (11%, 90% CI 0.6-43%). There were no unexpected safety signals and no discontinuations for toxicity. More than 70% of selected targets remained detectable by cell-free DNA despite a median of 6.5 months from biopsy to vaccination. Biomarker analysis suggested biologic activity in six of ten patients, and resistant clones showed diverse antigen-presentation defects.

Clinical bottom line: Personalized neoantigen vaccination is feasible and safe after platinum doublet in high-grade ovarian cancer. With ten patients and only biomarker activity endpoints, it belongs strictly in trials and has no place in clinic discussions of adjuvant options.

BEHOLD-1 and -2: phase 1/2 randomized studies evaluating the B7 homolog 4 protein-targeted antibody-drug conjugate mocertatug rezetecan in patients with solid tumors, including endometrial and ovarian cancers. — Lheureux Stephanie et al., Int J Gynecol Cancer

Mocertatug rezetecan (GSK5733584) links an anti-B7-H4 antibody to a topoisomerase 1 inhibitor payload through a protease-cleavable linker. BEHOLD-2 pairs it with dostarlimab in endometrial cancer and with bevacizumab in ovarian cancer. Planned enrollment is approximately 675 patients in BEHOLD-1 and approximately 305 in BEHOLD-2. Primary endpoints are dose-limiting toxicities in the dose-finding portions and confirmed objective response rate per RECIST v1.1 in the expansion portions.

Clinical bottom line: This is a trial-in-progress design paper, so its current value is referral. B7-H4 adds another ADC target for patients with platinum-resistant ovarian or mismatch repair proficient endometrial cancer who have exhausted standard options.

Genomic characterization of advanced and recurrent endometrial carcinoma: KRAS and TP53 co-mutation as a biomarker for adverse prognosis and therapeutic failure. — Lee Sanghoo et al., Gynecol Oncol

Whole-exome sequencing was performed on tumors from 67 patients, and deletions were more common in advanced disease (42.3% vs. 17.1%). Recurrent advanced cases had higher TP53 mutation rates (56.3% versus 10%) and lower PTEN mutation rates (25% versus 80%). KRAS mutation alone showed only a trend toward shorter OS (p=0.179), while KRAS-TP53 co-mutation had the worst survival (p=0.011). All co-mutated cases were chemoresistant, and among copy-number low and MSI-H tumors (n=46), co-mutation was strongly associated with adverse survival (p=0.003).

Clinical bottom line: In a 67-patient single series, KRAS plus TP53 co-mutation flags a uniformly chemoresistant subgroup of advanced endometrial cancer that may warrant trial referral rather than another platinum line. The numbers are too small to change adjuvant practice.

How these are chosen: filtered for clinical relevance, grouped by the new development discussed, then ordered by engagement (bookmarks and reposts).

Personalized neoantigen vaccine plus anti-PD-1 in high-grade ovarian cancer: feasible and safe in 10 patients, with activity measured only by biomarkers

NeoVax, a personalized peptide vaccine given with poly-ICLC and delayed nivolumab after platinum doublet, was feasible mainly at first recurrence rather than at new diagnosis. Selected targets largely persisted despite a median of 6.5 months of chemotherapy between biopsy and vaccination. Clinicians framed the result as an extension of neoantigen programs in melanoma, pancreatic cancer, glioblastoma, renal cell cancer and triple-negative breast cancer. Resistance through HLA loss and a JAK1 alteration, together with biomarker-only evidence of activity, leaves the next step open: a randomized post-platinum maintenance trial, or more work on antigen presentation first.

Key numbers: Vaccinated: Cohort B 75% (90% CI 47-93%) vs Cohort A 11% (90% CI 0.6-43%) · cfDNA-detectable targets >70% · Biomarker activity in six of ten patients

Molecular testing access gaps in endometrial cancer across Latin America: nearly half of centers report POLE testing is not offered

Noll et al. surveyed molecular testing and biomarker-guided endometrial cancer treatment across Latin America and found that POLE testing and treatment cost are both frequent gaps. Clinicians shared a case of a 72-year-old with HER2-positive (3+) metastatic uterine serous carcinoma. On a new systemic treatment, she reached complete response, stopped therapy at month 12 because of financial constraints, and maintained complete metabolic response at month 15. The open question is whether molecular-class-directed adjuvant therapy is a realistic global standard or a high-income-country recommendation.

Key numbers: POLE testing not offered 46.8% · Treatment cost cited as barrier 34%

TUB-040 responses independent of NaPi2b expression reopen the question of whether ovarian ADCs need a biomarker at all

According to Toon Van Gorp, responses to TUB-040 in platinum-resistant ovarian cancer did not differ by NaPi2b expression level. He argued that testing may be unnecessary because NaPi2b is expressed in more than 95% of ovarian cancers. Separately, next-generation ADCs were described as reaching response rates up to 50%+ in heavily pretreated disease, with many not requiring high biomarker expression. Against this, the Ann Oncol review lists biomarker complexity and tumor heterogeneity as major challenges. The debate is whether a registrational trial should drop biomarker selection entirely or retain it to guard against dilution of effect.

KEYLYNK-001 | Better PFS, But No Longer Survival?

An interesting phase III trial in advanced BRCA non-mutated ovarian cancer.

Part 1 — Why this trial?
Can combining immunotherapy with a PARP inhibitor improve outcomes?

Part 2 — The Good News
Pembrolizumab + olaparib improved median PFS by 7.6 months (22.2 vs 14.6 months).

But pembrolizumab alone did not help significantly.

Part 3 — The Surprise
Despite better PFS:

- No overall survival benefit demonstrated.
- More severe side effects.
- No meaningful improvement in quality of life.

Part 4 — The Big Question
There was no olaparib-only group.

So, did pembrolizumab really add anything beyond olaparib?

Take-home: A treatment can delay cancer progression without necessarily helping patients live longer.

And a positive trial can still leave important questions unanswered.

Ovarian Cancer NACT: Who, When & What Does the Evidence Really Say?

4 parts | Key clinical decisions & landmark evidence.

1/4 | Who Needs NACT?
Specific anatomical indications, surgical fitness and why FIGO stage alone is insufficient.

2/4 | Can Cytology Replace Biopsy?
CA-125/CEA >25, diagnostic safeguards and misclassification despite a presumed ovarian primary.

3/4 | Does NACT Compromise Survival?
EORTC 55971, CHORUS, JCOG0602 and SCORPION — including why JCOG0602 failed to establish noninferiority.

4/4 | Bevacizumab Before Surgery?
GOG-0218, ICON7, ANTHALYA and GEICO 1205 — separating adjuvant evidence from neoadjuvant extrapolation.

Key message: Evidence supports NACT in selected patients—not indiscriminate use.

RT @DrBarbiOnc: We treat rare cancers on guesses.

Stage IC clear cell endometrial cancer: no myoinvasion, negative cytology, missing from most trials, biology poorly understood.

New NCDB data ( n=188): adjuvant chemo vs none, HR for death 0.39 (CI 0.16 to 0.96). 5-yr OS 96.3% vs 87.7%.

Before anyone treats everyone:

Retrospective. A signal, not proof.
No molecular class, regimen, cycles or recurrence data.

Which molecular class benefits? Nobody knows yet.

Rare disease needs annotated, multi-institutional cohorts and trials that let these patients in.
