Sofetabart mipitecan, an antibody drug conjugate targeting folate receptor alpha, in high grade ovarian cancer: Results from the first-in-human phase 1 study. — Ray-Coquard I et al., Ann Oncol

The phase 1 study enrolled 105 patients with high-grade ovarian cancer, the majority platinum resistant, treated at 2, 3, 4 or 6 mg/kg every 21 days. The median number of prior treatments was 5.0, and 19 (18.1%) patients had prior mirvetuximab. Overall response rate was 50.5% (95% CI 40.5, 60.4) overall, 58.8% at 3 mg/kg, 44.4% for FRα <75%, 56.5% for FRα ≥75% and 57.9% after prior mirvetuximab. Median PFS was 8.1 months (95% CI 6.4, 8.5) at a median follow-up of 12.9 months. The most common TEAEs were nausea (64.8%), fatigue (56.2%) and anemia (47.6%), with no keratopathies or grade ≥3 neuropathy. Grade ≥3 treatment-related TEAEs occurred in 35.3% at 3 mg/kg and 56.4% at 4 mg/kg, and 11 patients (10.5%) discontinued for TEAEs.

Clinical bottom line: Sofetabart mipitecan is an investigational FRα ADC that showed activity irrespective of FRα expression level or prior mirvetuximab, without keratopathy. These data support taking 3 mg/kg into the phase 3 FRAmework-01 trial. It changes nothing in practice today, but it is the FRα agent to watch for patients who have already failed mirvetuximab.

Farletuzumab ecteribulin, a folate receptor alpha (FRα) antibody-drug conjugate, in select advanced cancers: An open-label, multicenter, phase 1/2 dose-finding study. — Wenham Robert M et al., Gynecol Oncol

The study enrolled eighty-nine patients (median age 67.0 years). Prior treatment was extensive: 88.7% of ovarian and 50.0% of endometrial cancer patients had received ≥2 prior regimens. ILD/pneumonitis occurred in 40% of patients in Dose-Escalation (n = 25), prompting body surface area-based dosing. In Dose-Confirmation (n = 24), ILD/pneumonitis occurred in 33.3% of patients; all events at 25 mg/m2 were grade 1/2, and the 33 mg/m2 dose was discontinued. Patients receiving 8 mg/m2 on D1/D8/D15 had no ILD/pneumonitis and more responses, though subgroups were small.

Clinical bottom line: Farletuzumab ecteribulin remains an investigational FRα ADC whose development depends on controlling a high rate of interstitial lung disease. Fractionated dosing is the current mitigation strategy, and there are no practice implications yet.

Platinum resistant ovarian cancer - No longer a graveyard for drug development. — Tewari Krishnansu S et al., Gynecol Oncol

The review states that platinum-resistant and refractory ovarian cancer is no longer regarded as a homogeneous entity now that targeted therapies have been tested in randomized trials. Over the past decade, five novel therapies spanning anti-angiogenesis, antibody-drug conjugates, checkpoint inhibition and glucocorticoid receptor blockade have become available, several of which confer a survival benefit. The authors identify adverse event mitigation and the role of biomarkers and their assays as issues to clarify.

Clinical bottom line: This review reframes platinum-resistant ovarian cancer as a set of biomarker-defined subpopulations rather than a single dead-end diagnosis. It is a useful citation for clinicians arguing for FRα and other biomarker testing at the time of resistance.

Neoadjuvant niraparib monotherapy for unresectable, homologous recombination deficiency-positive, high-grade serous ovarian cancer (NANT): a multicentre, single-arm, phase 2 trial. — Liu Dan et al., Lancet Oncol

The trial enrolled 67 patients with treatment-naive FIGO stage III-IV HRD-positive disease unsuitable for complete primary debulking, who received two 28-day cycles of niraparib at 200 mg or 300 mg. Among 48 response-evaluable patients, 30 responded (ORR 63%, 95% CI 47-76). Among 40 operated patients, 32 achieved R0 resection (80%, 95% CI 64-91). The most common grade 3-4 treatment-related adverse events were thrombocytopenia in 32 (48%), leukopenia in 13 (19%), anaemia in 12 (18%) and neutropenia in nine (13%). Serious adverse events occurred in 19 (28%) patients, with no treatment-related deaths, at a median follow-up of 37.7 months.

Clinical bottom line: Chemotherapy-free neoadjuvant niraparib produced responses and R0 resections in HRD-positive disease. This single-arm study does not displace platinum-taxane neoadjuvant chemotherapy, and the authors themselves call the strategy investigational pending randomised comparison.

PARAGON-II cohort C: a phase II basket study of letrozole plus ribociclib in hormone receptor-positive, platinum-resistant, high-grade epithelial ovarian cancer. — Lee Chee Khoon et al., Int J Gynecol Cancer

Thirty-two heavily pretreated participants were enrolled: 56% had ≥3 prior lines, 69% prior bevacizumab and 25% prior PARP inhibitors. Among 25 participants with measurable disease, ORR was 4.0% (95% CI 0.1 to 20.0; 1 partial response). In 30 evaluable participants, the 12-week disease control rate was 27.0%, median PFS was 1.9 months and the 6-month PFS rate was 16.0%. Grade ≥3 toxicities were mainly hematologic.

Clinical bottom line: Letrozole plus ribociclib should not be pursued in hormone receptor-positive, PIK3CA wild-type platinum-resistant ovarian cancer. The response rate was 4%, and the authors explicitly do not support further evaluation of the combination.

Early experience with mirvetuximab soravtansine in the treatment of recurrent low- grade serous ovarian carcinoma. — Tahan Elio et al., Int J Gynecol Cancer

This retrospective MD Anderson series included 12 patients with a median of 4 prior lines; 83.3% of tumors had high FRα expression (≥75% staining). Best responses were partial response in 4 (33.3%), stable disease in 7 (58.33%) and progressive disease in 1 (8.33%), for a disease control rate of 91.67%. Median duration of response was 8.37 months, and median PFS was not reached. Toxicities led to dose modifications in 58.33% of cases and discontinuation in 25% of patients.

Clinical bottom line: This 12-patient retrospective series supports trying mirvetuximab in heavily pretreated FRα-high recurrent low-grade serous ovarian carcinoma, a subtype excluded from the registrational trials. The small size and 25% discontinuation rate mean it should guide case-by-case decisions rather than standard practice.

New ASCO 2026 recurrent ovarian cancer guideline: FRα-high mirvetuximab gets a strong recommendation, trabectedin-PLD and routine secondary cytoreduction are out

The new ASCO 2026 guideline keeps a platinum doublet backbone (carboplatin plus PLD, paclitaxel or gemcitabine, with optional bevacizumab and maintenance) for platinum-sensitive recurrence and states not to add trabectedin to PLD. In platinum-resistant or refractory disease, mirvetuximab soravtansine is a strong recommendation for FRα-high tumors, which requires FRα testing. Secondary cytoreduction is not routinely recommended, and HIPEC evidence remains insufficient, even as clinicians call the binary platinum "line in the sand" outdated. The tension is a separate 152-patient, 31-center study linking secondary cytoreduction in recurrent low-grade serous disease to longer survival (adjusted HR 0.28; p<0.001), with complete resection in 92%.

Update: KEYLYNK-001 lands in The Lancet Oncology and the conversation narrows to a PFS win with no overall survival benefit

In phase III KEYLYNK-001, reported by Ignace Vergote in The Lancet Oncology, pembrolizumab plus chemotherapy followed by pembrolizumab/olaparib improved PFS in newly diagnosed BRCA-nonmutated advanced ovarian cancer, but overall survival was not improved. The same week, clinicians debated changing concepts of platinum-sensitive relapse, the role of antibody-drug conjugates and therapeutic sequencing. The open question is whether a PFS-only gain earns pembrolizumab a place in any frontline ovarian regimen.

FRα antibody-drug conjugates after mirvetuximab: sofetabart mipitecan and rinatabart sesutecan report activity independent of FRα level

Sofetabart mipitecan, an Fc-silent FRα antibody linked to exatecan, produced responses overall and after prior mirvetuximab without keratopathies or grade ≥3 neuropathy. Grade ≥3 treatment-related toxicity rose with dose. Rinatabart sesutecan likewise showed efficacy regardless of FRα status in heavily pretreated platinum-resistant disease, though clinicians note there is still no answer for ADC after ADC. The debate is whether FRα immunohistochemistry stays a gating biomarker or becomes purely a label requirement for mirvetuximab.

Key numbers: Sofe-M ORR 50.5% (95% CI 40.5, 60.4) · ORR after prior MIRV 57.9% · Median PFS 8.1 mo · Grade ≥3 TRAEs 35.3% (3 mg/kg) vs 56.4% (4 mg/kg) · Rina-S 5 complete responses

NANT: chemotherapy-free neoadjuvant niraparib gets 63% response and 80% R0 in HRD-positive ovarian cancer, but attrition and single-arm design keep it investigational

Neoadjuvant niraparib monotherapy given to patients with unresectable HRD-positive disease produced responses in most evaluable patients and R0 resection in most of those operated, with higher activity in BRCA-mutated tumors. Thrombocytopenia was the dominant toxicity, and substantial attrition before response assessment weakens the signal. Both commentators and the authors call the strategy investigational, requiring randomised comparison with platinum-taxane chemotherapy. Whether a chemotherapy-free window is defensible outside a trial, even in BRCA-mutated disease, remains contested.

Key numbers: ORR 63% (30/48) · R0 80% (32/40 operated) · BRCA-mutated ORR 77% · Grade ≥3 thrombocytopenia 48% · Discontinued before response assessment 28%

RT @DrRishabhOnco: 🚨 NEW ASCO 2026 GUIDELINE: Recurrent Ovarian Cancer

A practical treatment algorithm worth saving 👇

PLATINUM-SENSITIVE recurrence
→ Platinum doublet remains backbone:
• Carboplatin + PLD
• Carboplatin + paclitaxel
• Carboplatin + gemcitabine
→ Bevacizumab may be added, followed by maintenance bevacizumab
→ Do NOT add trabectedin to PLD

PLATINUM-RESISTANT / REFRACTORY
⭐ FRα-high: Mirvetuximab soravtansine = strong recommendation
Other options:
• PLD
• Paclitaxel
• Bevacizumab + chemotherapy
• Relacorilant + nab-paclitaxel
• Gemcitabine

SURGERY
Secondary cytoreduction is not routinely recommended based on current evidence, but may be discussed when complete gross resection is highly achievable.

HIPEC
Evidence remains insufficient for a routine recommendation.

Key message:
Recurrent ovarian cancer is becoming increasingly biomarker-driven and patient-selected, especially in platinum-resistant disease.

RT @DrRishabhOnco: 🚨 Can PARP inhibitors replace neoadjuvant chemotherapy in ovarian cancer?

NANT | Phase II | Lancet Oncology 2026

67 patients with unresectable, HRD+ advanced ovarian cancer received neoadjuvant niraparib monotherapy for 8 weeks.

📊 Key Results
• ORR: 63% (30/48)
• R0 resection: 80% (32/40 operated)
• BRCA-mutated ORR: 77%
• Median PFS: 19.7 months
• Median OS: 53.2 months

⚠️ Grade ≥3 thrombocytopenia: 48%
28% discontinued before response assessment.

💡 Verdict: Promising proof of concept for chemotherapy-free neoadjuvant PARP inhibition, particularly in BRCA-mutated disease.

But this is a single-arm study with substantial attrition. Not practice-changing yet. Randomised comparison with platinum-taxane chemotherapy is essential.
