Comparing niraparib versus platinum-taxane doublet chemotherapy as neoadjuvant treatment in patients with newly diagnosed homologous recombination-deficient stage III/IV ovarian cancer: Findings from cohort C of the OPAL phase 2 trial. โ€” Westin Shannon N et al., Gynecol Oncol

Of 36 patients randomized, 19 received niraparib and 17 platinum-taxane. The pre-IDS unconfirmed ORR was 31.6% with niraparib versus 70.6% with platinum-taxane, an ORR difference of -39.0% (80% CI -56.8% to -17.7%) favoring platinum-taxane. Grade โ‰ฅ3 adverse-event rates during the neoadjuvant period were 42% and 59% in the niraparib and platinum-taxane arms, respectively, and the trial was closed early for futility.

Clinical bottom line: The full OPAL cohort C publication confirms neoadjuvant niraparib should not replace platinum-taxane chemotherapy in newly diagnosed HRd advanced ovarian cancer, with a 39-percentage-point ORR deficit that closed the trial for futility.

Molecular Response Assessment From Circulating Tumor DNA in Patients With Ovarian Cancer Treated With the WEE1 Inhibitor Azenosertib. โ€” Jeong Jinkil et al., JCO Precis Oncol

Plasma cell-free DNA was collected at baseline and on-treatment from 123 patients with recurrent HGSOC enrolled in clinical trials of azenosertib; molecular response (MR) was defined as a reduction of TP53 variant allelic fraction >50% at the earliest evaluable on-treatment time point. Molecular responders showed a significantly longer time to progression than nonresponders (median 5.49 vs 2.69 months, HR = 0.43), and MR identified patients with prolonged survival among those with stable disease at first radiographic assessment (HR = 0.44). MR was evaluable in 88% (102 of 116) of patients versus 71% (85 of 116) for CA-125, with similar predictive value (HR = 0.41 vs 0.45 for time-to-progression evaluation).

Clinical bottom line: ctDNA-based molecular response using TP53 variant allelic fraction is evaluable in more patients than CA-125 and separates responders from nonresponders weeks before imaging, making it a usable early read-out in azenosertib trials for high-grade serous ovarian cancer.

Secondary cytoreduction for ovarian cancer relapse: results from the high surgical quality, multi-center "Surgery in Recurrent Ovarian Cancer Research Consortium (SURROC-CON)". โ€” du Bois Andreas et al., Int J Gynecol Cancer

Of 3109 included patients, 38.7% underwent secondary cytoreduction and 83.6% achieved complete clearance; median overall survival was 81 months (95% CI 73 to 88) after complete clearance versus 39 months (95% CI 33 to 44) with residual disease (p <.001), compared with 34 months (95% CI 33 to 36) with chemotherapy alone. BRCA1/2 mutations were associated with improved progression-free survival after secondary cytoreduction (27.4 vs 21.3 months, p <.0021) but not overall survival. Median OS after surgery was 73 months (95% CI 65 to 79) without prior maintenance versus 55 months (95% CI 47 to 63) after prior bevacizumab versus 44 months (95% CI 32 to 56) after prior PARP inhibitors (p =.0056).

Clinical bottom line: In a 3109-patient multi-center registry from high-volume centers, secondary cytoreduction achieving complete gross resection nearly doubled median overall survival versus chemotherapy alone, and prior PARP inhibitor or bevacizumab maintenance โ€” not BRCA status or timing of primary surgery โ€” marked the patients who did worse after surgery.

Clinical outcomes and prognostic factors in advanced high-grade platinum-resistant ovarian carcinoma: evidence from the ESME real-world cancer database. โ€” Heurtier Victor et al., Int J Gynecol Cancer

A total of 688 patients were included: 580 (84.3%) received chemotherapy alone and 108 (15.7%) chemotherapy plus bevacizumab; median progression-free survival and overall survival were 3.2 and 8.8 months overall. In the chemotherapy-alone group, gemcitabine was associated with inferior PFS versus paclitaxel (2.5 vs 3.4 months; HR 1.31, 95% CI 1.03 to 1.67), while in the bevacizumab group, median PFS was 6.5 months with paclitaxel plus bevacizumab compared with 4.1 months with pegylated liposomal doxorubicin plus bevacizumab (HR 3.50, 95% CI 1.93 to 6.34) and 3.6 months with gemcitabine plus bevacizumab (HR 2.54, 95% CI 1.10 to 5.88). In the overall multivariable model, bevacizumab was independently associated with improved overall survival (HR 0.73, 95% CI 0.56 to 0.94).

Clinical bottom line: In 688 real-world platinum-resistant patients, paclitaxel was the better chemotherapy backbone โ€” especially with bevacizumab โ€” while the apparent bevacizumab survival advantage should be read as confounding by indication rather than proof of benefit.

Meta-analysis of 5 phase III trials finds no overall survival benefit from whole-pelvis radiotherapy

A meta-analysis of 5 phase III trials including 5,172 patients found no OS benefit with whole-pelvis radiotherapy, and the authors conclude that routine elective pelvic irradiation is not supported in unselected patients. Set against this, an IJGC study of a radiotherapy boost to positive nodes in stage IIIC cervical cancer reported improved OS of 107 vs 91 months, particularly in patients with smaller or fewer nodes. The contrast between these datasets frames a direct question for radiation oncologists: if elective whole-pelvis irradiation adds no survival benefit in unselected patients, should nodal fields be reserved for image-defined nodal disease where boost data show a survival signal?

SURROC-CON and the trial record converge: secondary cytoreduction only pays when resection is complete

In the SURROC-CON consortium of 3109 patients, 38.7% underwent secondary cytoreduction and 83.6% achieved complete clearance, yielding a median overall survival of 81 months versus 39 months with residual disease and 34 months with chemotherapy alone. The randomized trial record reinforces this pattern: DESKTOP III reported median OS of 53.7 vs 46.0 months overall, with complete resection achieved in 75.5% of patients who then lived a median of 61.9 months, while a separate summary using the AGO selection algorithm notes that when complete resection was achieved, median OS was 62 months versus 28 months with incomplete resection (HR 0.75). The divergent results across trials are attributed to selection methodology โ€” DESKTOP III used AGO selection and demonstrated an overall survival benefit, SOC-1 used iMODEL plus PET-CT and showed a clear PFS benefit without significant ITT OS improvement, and GOG-0213 used investigator-selected resectability and showed no overall survival benefit. Given that prior PARP inhibitor or bevacizumab maintenance marked shorter survival after secondary surgery in SURROC-CON, specialists are debating whether maintenance exposure should now enter the selection algorithms alongside AGO and iMODEL criteria.

ASCO issues its first Living Guideline on systemic treatment of recurrent ovarian cancer

ASCO published its inaugural guideline on systemic treatment of recurrent ovarian cancer under the title Systemic Treatment of Ovarian Cancer Recurrence: ASCO Living Guideline, Version 2026.1.0. ASCO experts Dr. Lesnock of UPMC and Dr. Scalici of Winship at Emory have recorded an explanation of the new guideline and the data behind its recommendations. The living format invites the question of which recurrence decisions โ€” platinum-sensitive re-treatment, PARP inhibitor re-challenge, or ADC sequencing โ€” the guideline will need to revise first.

๐Ÿงฌ #Endometrialcancer was once classified by histology alone. Now it's molecular: POLE-ultramutated, dMMR, NSMP, p53-abnormal.

New biomarker data from the DUO-E trial is helping untangle the toughest subgroup of all โ€” pMMR โ€” paving the way for smarter, more targeted trials ahead.

The p53wt #Endometrialcancer selection in #XPORT-EC-042 made sense. #Selinexor blocks XPO1 and keeps wild-type p53 in the nucleus, so the drug needs working p53 to do anything. Mutated tumors give it nothing to hit. #SIENDO's p53wt subgroup looked like proof, HR 0.41. So why is the trial negative? (12.75 vs 7.43 mo, HR 0.76, 95% CI 0.51-1.12, topline) My read: p53 WT tells you the target is there, not that the drug can reach it. Reactivated p53 kills a dividing, stressed cell. Maintenance after platinum leaves quiet, resting disease. The biomarker got the target right. But the maintenance setting, post-platinum, hands the drug a cell it can't act on. Right target, wrong state.#gyncsm
