Persistent low-level real human chorionic gonadotropin after gestational trophoblastic disease: Natural history and predictors of subsequent gestational trophoblastic neoplasia. β€” Alevato Raphael et al., Gynecol Oncol

Among 7304 patients with GTD, 72 (0.9%) developed persistent low-level real hCG. Spontaneous normalization occurred in 88.4% (38/43) during postmolar surveillance and 75.9% (22/29) after GTN remission. During postmolar surveillance, vaginal hemorrhage (RD: 35.7%, 95% CI: 10.6–60.8), preevacuation hCG β‰₯100,000 IU/L (RD: 22.7%, 95% CI: 5.2–40.2), and delayed onset of persistent low-level real hCG β‰₯7 weeks (RD: 16.6%, 95% CI: 3.3–30.0) were associated with increased risk of postmolar GTN; after GTN remission, delayed onset β‰₯14 weeks carried an RD of 43.8% (95% CI: 19.4–68.1) for relapsed GTN.

Clinical bottom line: Persistent low-level real hCG after gestational trophoblastic disease usually resolves on its own, so surveillance until FIGO criteria are met β€” not immediate chemotherapy β€” is the right default, with hemorrhage, high preevacuation hCG, and delayed onset flagging the patients who will need treatment.

The pan-tumor landscape, allelic status, and genomic complexity of SMARCA4 alterations. β€” Repetto Matteo et al., Clin Cancer Res

This study analyzed 68,920 tumor-normal paired samples sequenced with MSK-IMPACT between 2015 and 2023. SMARCA4 alterations were most frequent in thymic epithelial tumors (10.7%), NSCLC (7.1%), bladder cancer (5.6%), and cervical cancer (5%), with class 1 and class 2 alterations occurring in 2.9% and 0.7% of tumors, respectively. In a phase I trial of the BRM degrader PRT3789, patients with monoallelic class 2 SMARCA4 alterations and preserved BRG1 expression saw clinical benefit, including one complete response.

Clinical bottom line: Reporting a SMARCA4 alteration is not enough β€” cervical cancer carries these alterations in 5% of cases, and allelic status plus functional mutation class should drive who is selected for BRM-targeted agents such as PRT3789.

AMH as a marker of chemotherapy benefit: cytotoxicity or ovarian suppression?

AMH may indicate who benefited from chemotherapy, but not whether the mechanism is direct cytotoxicity or ovarian suppression β€” a distinction that matters when interpreting biomarker-stratified outcomes. Until optimal ovarian function suppression plus endocrine therapy is directly compared with chemotherapy, the role of chemotherapy remains debatable. Separately, clinicians noted that Lynch syndrome surveillance should be gene-specific, not one-size-fits-all, and should balance cancer risk with fertility goals. The core question is whether any biomarker of ovarian reserve can settle the mechanism debate short of a head-to-head trial against ovarian function suppression plus endocrine therapy.

GLP-1 receptor agonist users show a lower endometrial cancer hazard in a real-world obesity cohort

In obese adults without diabetes, obesity-associated cancer occurred in 0.54% vs 0.90% (HR 0.59), with endometrial cancer among the strongest signals at HR 0.42 β€” alongside multiple myeloma (HR 0.37), pancreatic cancer (HR 0.40), and colorectal cancer (HR 0.49). Median follow-up was only approximately 2 years, and the study was observational rather than randomized, limiting causal inference. Oncologists also noted that an oral anticoagulation quality improvement program successfully decreased VTE events during first-line chemotherapy for ovarian cancer. The unresolved question is what trial design would be required before GLP-1 agents are discussed as risk-reduction tools in obese patients, given an endometrial cancer HR of 0.42 from two years of observational follow-up.

FDA Fast Track for BI-1808 plus pembrolizumab in platinum-resistant ovarian cancer, on a 24% confirmed response rate

The FDA granted Fast Track designation to BI-1808, an anti-TNFR2 agent, in combination with pembrolizumab for patients with advanced ovarian cancer, based on interim phase 2a data in which the chemotherapy-free combination produced a 24% confirmed ORR in pretreated platinum-resistant ovarian cancer. The designation rests on responses seen in heavily pretreated patients with platinum-resistant disease, without the addition of chemotherapy. Clinicians are weighing whether a 24% confirmed response rate from interim phase 2a data is sufficient to position TNFR2 blockade combined with pembrolizumab as a serious contender in this setting.

#Endometrialcancer isn't one disease. We now sort it into four molecular groups, and the group drives the treatment.
1 #POLE-mutated: excellent prognosis, we're starting to give less.
2 #MMRd (mismatch repair deficient): the group that responds to immunotherapy.
3 #p53-abnormal: the most aggressive, treated hardest.
4 #NSMP: no specific marker, the default.
The groups are meant to be mutually exclusive. But POLE, MMRd, and abnormal p53 can show up in the same tumor. When they do, a rule ranks them, POLE over MMRd over p53, and we keep the top one and set the rest aside. That rule came from small retrospective series where the lower marker looked like a passenger. Fair. But it's a population average turned into a blanket rule, applied to the one tumor in front of you. We don't drop the second marker because we measured it and found it silent here. We drop it because the hierarchy said so. It's sharpest with MMRd, the one group that's also predictive. When POLE outranks it, we file the tumor as POLE and lose the result that would have pointed to immunotherapy. The rule is probably right most of the time. The honest problem is that it's built to never ask when it's wrong.

The p53wt #Endometrialcancer selection in #XPORT-EC-042 made sense. #Selinexor blocks XPO1 and keeps wild-type p53 in the nucleus, so the drug needs working p53 to do anything. Mutated tumors give it nothing to hit. #SIENDO's p53wt subgroup looked like proof, HR 0.41. So why is the trial negative? (12.75 vs 7.43 mo, HR 0.76, 95% CI 0.51-1.12, topline) My read: p53 WT tells you the target is there, not that the drug can reach it. Reactivated p53 kills a dividing, stressed cell. Maintenance after platinum leaves quiet, resting disease. The biomarker got the target right. But the maintenance setting, post-platinum, hands the drug a cell it can't act on. Right target, wrong state.#gyncsm
