Poly(adenosine diphosphate-ribose) polymerase inhibitor maintenance therapy with niraparib in patients with primary advanced or recurrent endometrial cancer receiving dostarlimab plus chemotherapy. β€” Powell Matthew A et al., Int J Gynecol Cancer

In the RUBY Part 2 analysis, the risk of progression or death was significantly reduced by 40% (HR 0.60, 95% CI 0.43–0.82, p<.001) in the overall population and by 37% (HR 0.63, 95% CI 0.44–0.91, p=.006) in the mismatch repair-proficient/microsatellite stable population with niraparib plus dostarlimab versus control. At 36.2 months of follow-up, no overall survival benefit was observed (HR 1.2, 95% CI 0.81–1.78). Grade β‰₯3 treatment-related adverse events occurred in 70.7% of patients in the niraparib plus dostarlimab arm versus 37.5% in the control arm, and serious treatment-related adverse events occurred in 24.6% and 9.4%, respectively.

Clinical bottom line: Adding niraparib to dostarlimab maintenance improved PFS but produced no OS benefit and substantially more toxicity, so dostarlimab plus chemotherapy followed by dostarlimab maintenance remains the standard in advanced/recurrent endometrial cancer.

Maintenance therapy in gynecologic malignancies: Current and future state. β€” Widick Page C et al., Cancer

This review describes how PARP inhibitors demonstrate particularly robust results in patients with BRCA-mutated disease or other homologous recombination-deficient cancers. In endometrial and cervical cancers, immune checkpoint inhibitors combined with chemotherapy and used as maintenance thereafter have improved PFS and OS, with particular benefits noted in mismatch repair-deficient endometrial cancer and PD-L1-positive cervical cancer.

Clinical bottom line: Maintenance therapy with VEGF and PARP inhibitors in ovarian cancer and checkpoint inhibitors in endometrial and cervical cancers can improve PFS and sometimes OS, but must be weighed against added toxicity, cost, and quality-of-life impact.

FIGO 2023 staging may result in overtreatment of early-stage endometrial cancer. β€” Roy Allison et al., Gynecol Oncol

This analysis finds that upstaging under FIGO 2023 may result in increased treatment β€” including additional chemotherapy and radiation β€” with unclear impact on cancer outcomes. Neither the NCCN guidelines nor the literature on which treatment is based address the best treatment for patients who are upstaged.

Clinical bottom line: This analysis quantifies that FIGO 2023 upstaging of early endometrial cancer risks additional chemotherapy and radiation, so clinicians should be cautious applying literature-based recommendations to upstaged patients.

Technical evaluation of commercially available homologous recombination deficiency assays using MyChoice CDx as reference in ovarian cancer. β€” Yarunin Alexander et al., Gynecol Oncol

Using MyChoice CDx as reference, overall percentage agreement for HRD status ranged 76.0%–88.7%; positive percentage agreement ranged 81.6%–93.8%, and negative percentage agreement ranged 70.3%–90.4% across commercially available assays. Among simulated HRD-positive participants, mean progression-free survival hazard ratio was 0.40 at PPA of 80% and NPA of 85%, and 0.38 at PPA of 75% and NPA of 90%.

Clinical bottom line: Commercially available HRD assays showed acceptable concordance with MyChoice CDx for HRD, genomic instability, and BRCA mutation detection, supporting their potential clinical utility for treatment selection in ovarian cancer.

EU approves tumour-agnostic trastuzumab deruxtecan, with implications for HER2 IHC testing in gynecologic tumors

Trastuzumab deruxtecan received EU approval as the first tumour-agnostic HER2-directed antibody-drug conjugate for previously treated, unresectable or metastatic HER2-positive (IHC 3+) solid tumours without satisfactory options. Because the approval is based on HER2 IHC 3+, which is not captured by NGS, oncologists will need to proactively request HER2 IHC for patients potentially eligible for trastuzumab deruxtecan β€” including those with HER2-positive gynecologic tumors. The central question for specialists is how HER2 IHC testing should be integrated into the standard workup for gynecologic tumors now that this tumour-agnostic indication is available in Europe.

RT @PTarantinoMD: Finally histology agnostic T-DXd reaches Europe!

Key fact to remember: the approval is based on HER2 IHC 3+, which is not captured by NGS β€” thus, it will be critical to proactively request HER2 IHC for patients potentially eligible for T-DXd (eg HER2+ GYN tumors, mUC, mCRC…)
