Talazoparib plus enzalutamide in high-volume metastatic hormone-naive prostate cancer: a randomized phase 2 trial. โ€” Zacchi Francesca et al., Nat Med

Fifty-four patients starting androgen deprivation therapy received two cycles of enzalutamide lead-in, then were randomized 1:2 to enzalutamide (n = 17) or enzalutamide plus talazoparib (n = 37). The primary endpoint was met: the PSA <0.2 ng/ml response rate at 12 months was 73% in the combination arm (95% CI = 56-86%, P < 0.001 against the prespecified threshold), compared with 64.7% in the control arm (95% CI = 38-86%, P = 0.54 for the exploratory between-arm comparison). Grade โ‰ฅ3 adverse events occurred in 68% with the combination versus 35% with enzalutamide alone, predominantly anemia (41%). Two fatal therapy-emergent myeloid neoplasms occurred in the combination arm in patients with underlying genomic susceptibility. Paired biopsies showed suppression of HRR-related gene expression after AR inhibition without functional impairment of homologous recombination.

Clinical bottom line: In biomarker-unselected high-volume metastatic hormone-naive prostate cancer, adding talazoparib to enzalutamide added toxicity, including two fatal myeloid neoplasms, without clear benefit over enzalutamide alone. The combination should stay out of practice outside molecularly selected populations.

Neoadjuvant Stereotactic Ablative Radiotherapy Before Inferior Vena Cava Thrombectomy for Renal Cell Carcinoma: A Phase 2 Nonrandomized Clinical Trial. โ€” Hannan Raquibul et al., JAMA Oncol

Patients with RCC and Mayo level II to IV IVC tumor thrombus received SABR to the thrombus, delivered as 40 Gy in 5 fractions or 36 Gy in 3 fractions, before radical nephrectomy and IVC thrombectomy. Of 23 patients treated, 22 completed surgery and 21 were analyzed. One-year RFS was 71.4% (80% CI, 56.6-82.0; 95% CI, 47.2-86.0), which met the screening criterion against a historical surgery-alone benchmark of 55.7% (P = .09). One-year overall survival was 95.2%. There was no 90-day perioperative mortality, and 4 grade 4 treatment-attributed events occurred, with no grade 4/5 radiation-attributed toxic effects.

Clinical bottom line: Neoadjuvant SABR to IVC tumor thrombus is feasible and safe in this single-center phase 2. With only 21 analyzable patients and a historical control, it does not change surgical practice today for RCC with venous thrombus.

FDA expands Anktiva plus BCG to BCG-unresponsive high-grade papillary-only NMIBC on extrapolation from CIS data

FDA expanded Anktiva (nogapendekin alfa inbakicept-pmln) with BCG to BCG-unresponsive high-grade papillary-only NMIBC, the first approval specifically for this group. The FDA concluded that efficacy shown in CIS-containing disease could be extrapolated to papillary disease based on overlapping features. Clinicians flagged the nuance: the complete response figure comes from CIS patients, and both cohorts came from the same single-arm trial without a randomized papillary-only comparison. The open question is whether that extrapolation is enough to choose Anktiva plus BCG over cystectomy or a competing intravesical agent.

Update: ESMO26 GU preview sharpens the perioperative MIBC question โ€” VOLGA, ENERGIZE and KEYNOTE-866 versus the EV-pembro benchmark

Four randomized phase III perioperative bladder trials are at ESMO26: VOLGA, KEYNOTE-866, ENERGIZE and KEYNOTE-905. ctDNA data from KEYNOTE-B15/EV-304 will also be presented, while no personalized cancer vaccine or RT +/- ICI data are on the program. VOLGA has a positive EFS/OS press-release topline for durvalumab plus EV, but the magnitude, surgical outcomes and toxicity are still awaited. Clinicians ask whether VOLGA's shorter EV exposure matches perioperative EVP, where longer EV gave an OS HR of 0.5-0.6, and whether ENERGIZE and KEYNOTE-866 reproduce NIAGARA's OS HR of 0.75 or show antagonism with platinum. The underlying trade-off is whether post-op EV can be omitted for a less toxic paradigm; as one clinician cautioned, identifying a favorable-risk group by ctDNA is different from proving treatment can be safely reduced.

Update: FDA approval of perioperative EV plus pembrolizumab for cisplatin-ineligible MIBC is followed by a CHMP positive opinion in Europe

Following FDA approval of EV-pembrolizumab for cisplatin-ineligible resectable MIBC based on KEYNOTE-B15/EV-304, CHMP adopted a positive opinion recommending the regimen as neoadjuvant therapy. The trial improved EFS, OS and pathologic complete response, with rash and neuropathy the notable adverse events. The practical debate centers on patients with minimal response to neoadjuvant EV-pembro: should adjuvant therapy continue the same regimen or switch to chemotherapy?

Multi-institutional analysis suggests adjuvant pembrolizumab benefit in clear cell RCC scales with baseline recurrence risk

A multi-institutional study of 739 KEYNOTE-564-eligible patients suggests that adjuvant pembrolizumab benefit in clear cell RCC may scale with baseline recurrence risk, although the interaction was not significant. Final OS for KEYNOTE-564 is among the ESMO26 renal highlights. Clinicians are weighing whether to reserve adjuvant pembrolizumab for the highest-risk patients, or whether a non-significant interaction means it should still be offered to all eligible patients.

LUNAR at 32 months: 177Lu-PNT2002 before SBRT doubles PFS in oligorecurrent hormone-sensitive prostate cancer and now improves castrate disease-free survival

The phase II LUNAR trial tested 177Lu-PNT2002 before SBRT in oligorecurrent hormone-sensitive prostate cancer. Among 92 randomized patients, median PFS improved from 7.4 to 17.6 months. At a median follow-up of 32 months, exploratory analyses show improvement in polymetastatic PFS, castrate disease-free survival and CT-based PFS. The debate is whether the PFS gain justifies use now, or whether castrate disease-free survival should first be confirmed outside exploratory analysis.

ZZ-FIRST published in Nature Medicine: talazoparib plus enzalutamide adds toxicity without clear benefit in high-volume hormone-naive prostate cancer

ZZ-FIRST, an academic randomized phase II trial with spatial transcriptomics on longitudinal biopsies, met its PSA response endpoint in the combination arm, but superiority over enzalutamide alone was not established. Commentators noted no significant rPFS/OS benefit, greater toxicity including two fatal cases of AML, and the small size of the trial (n=54). Tumor analyses showed that AR inhibition altered gene expression without impairing functional homologous recombination in this biomarker-unselected population. Clinicians argue for molecular selection over indiscriminate intensification, which raises the question of whether PARP inhibitor plus ARPI combinations should be restricted to HRR-altered patients.

ESMO26 renal preview: negative frontline triplet press release and HIF-2alpha positioning after IO

A press release reports that the frontline triplet comparison of lenvatinib/pembrolizumab/belzutifan versus lenvatinib/pembrolizumab/CTLA4 versus lenvatinib/pembrolizumab is negative. LITESPARK-012 also missed its PFS/OS endpoints when belzutifan was added to frontline pembrolizumab/lenvatinib. In contrast, LITESPARK-011 improved PFS with belzutifan plus lenvatinib versus cabozantinib after IO, leading to September FDA approval, though final OS was not significant. ESMO will add QoL analyses and prior-treatment subgroups, which may help locate where HIF-2alpha inhibition adds value given anemia/hypoxia and TKI toxicity.

The FDA has expanded the approval of nogapendekin alfa inbakicept-pmln with BCG for BCG-unresponsive non-muscle invasive bladder cancer to include patients with high-grade papillary-only tumors.

Based on the overlapping features of high-grade papillary and carcinoma in situ tumors, the FDA concluded that the prior demonstration of efficacy in disease containing CIS could be extrapolated to BCG-unresponsive high-grade papillary disease.

FDA expands Anktiva + BCG approval for BCG-unresponsive non-muscle-invasive bladder cancer to include high-grade papillary-only tumors. Previously, the indication required CIS (carcinoma in situ).

Original CIS cohort (n=77): 62% complete response, with 40% of responders maintaining their response for at least 2 years.

Papillary-only cohort (n=80): 58% disease-free at 1 year.

Both cohorts came from the same single-arm trial, with no randomized comparison. The expansion relied on existing data plus the biological overlap between CIS and papillary disease. So, another option to preserve the bladder.
