Enfortumab Vedotin and Pembrolizumab in Cisplatin-Eligible Bladder Cancer. — Galsky Matthew D et al., N Engl J Med
In 405 participants assigned to enfortumab vedotin-pembrolizumab and 403 to cisplatin-gemcitabine, 2-year event-free survival was 79.4% versus 66.2% (HR 0.53; 95% CI, 0.41 to 0.70; P<0.001) and estimated overall survival was 86.9% versus 81.3% (hazard ratio for death, 0.65; 95% CI, 0.48 to 0.89; two-sided P = 0.006). Pathological complete response occurred in 55.8% versus 32.5% of participants (P<0.001), while grade 3 or higher adverse events of any cause were reported in 75.7% versus 67.2%.
Clinical bottom line: The full KEYNOTE-B15/EV-304 publication in NEJM supplies the primary data behind the FDA approval, showing perioperative enfortumab vedotin plus pembrolizumab beats neoadjuvant cisplatin-gemcitabine on event-free survival, overall survival, and pathological complete response at the cost of more grade 3 or higher toxicity, making it the new standard for cisplatin-eligible muscle-invasive bladder cancer.
Multikinase Inhibition with Tinengotinib in Advanced Solid Tumors: Results from a Multicenter Phase Ib/II Trial. — Piha-Paul Sarina A et al., Clin Cancer Res
Among 197 patients receiving tinengotinib, treatment-related treatment-emergent adverse events occurred in 160 (81.2%) patients, most commonly hypertension. The objective response rate was 33.3% in the prostate cancer cohort (n=15), and clinical benefit was achieved by 33/159 (20.8%) evaluable patients, including 1 complete response and 20 partial responses. Exploratory analyses suggested MYC alterations in prostate cancer were associated with shorter progression-free survival.
Clinical bottom line: Tinengotinib produced a 33.3% response rate in a small prostate cancer cohort with little hyperphosphatemia or ocular toxicity, and MYC alterations marked shorter progression-free survival, supporting biomarker-informed development in advanced prostate cancer.
Randomized phase II trial of nivolumab and ipilimumab with or without stereotactic body radiation therapy in patients with metastatic castration-resistant prostate cancer: the CheckPRO, CA209-8TY trial. — Eefsen Rikke Løvendahl et al., J Immunother Cancer
The confirmed PSA response rate was 21.6% (95% CI 9.8% to 38.2%) in arm A (SBRT plus nivolumab/ipilimumab) and 20.5% (95% CI 9.8% to 35.3%) in arm B, with objective response rates of 16.7% (95% CI 4.7% to 37.4%) and 22.2% (95% CI 10.1% to 39.2%), respectively. Median OS was 10.2 months (95% CI 7.1 to 15.2) in arm A and 9.2 months (95% CI 7.1 to 14.4) in arm B. Grade 3–4 treatment-related adverse events were observed in 27 patients (33.3%).
Clinical bottom line: Adding SBRT to nivolumab plus ipilimumab did not improve PSA response, objective response, or survival in pretreated mCRPC, so radiation-primed checkpoint blockade should not be pursued in this setting without a predictive biomarker.
Safety and efficacy of fecal microbiota transplantation in solid cancers resistant to immune checkpoint inhibitors: results of the MITRIC trial. — Ullern Andreas et al., J Immunother Cancer
The MITRIC trial enrolled 12 patients, including 1 with renal cell carcinoma; no objective responses were observed, while 5/12 patients recorded stable disease. Median PFS was 1.5 months and median OS was 10.1 months. Fecal microbiota transplantation was well tolerated, with immune-related toxicity occurring in 6/12 patients.
Clinical bottom line: Fecal microbiota transplantation added to checkpoint inhibitors was safe and engrafted in most patients but produced no objective responses, so microbiome manipulation for ICI-refractory solid tumors including RCC still needs donor selection and patient selection criteria before it can be used clinically.
How these are chosen: filtered for clinical relevance, grouped by the new development discussed, then ordered by engagement (bookmarks and reposts).
Perioperative enfortumab vedotin plus pembrolizumab delivered 2-year EFS of 79.4% versus 66.2% (HR 0.53), OS of 86.9% versus 81.3% (HR 0.65; 95% CI 0.48–0.89; P=0.006), and pCR of 55.8% versus 32.5%, with grade ≥3 adverse events in 75.7% versus 67.2%. Commentators note that after more than 20 years, a non-platinum perioperative regimen beats cisplatin-based chemotherapy in resectable MIBC, and that every enfortumab vedotin phase III study to date has improved OS versus standard of care. The clinical debate has moved from whether to use EV plus pembrolizumab to how to use it — who truly benefits, what optimal management after pCR looks like, and whether bladder preservation is feasible in selected responders — while the efficacy gains must be weighed against higher toxicity and issues of access and cost.

The updated NICE 2026 guideline sets out a comprehensive pathway from suspicion to risk-adapted treatment and follow-up, against the backdrop that 87% of stage 1a RCC in the UK is diagnosed incidentally. A systematic review and meta-analysis from the EAU YAU renal cancer group characterises renal tumour biopsy as having a yield of 90%, sensitivity of 99%, complication rate of 3%, and changing management in 39% of cases. The central question is what role structured biopsy pathways and risk-adapted follow-up should play when the dominant mode of early RCC detection remains incidental imaging.

Three quality-of-life tools — FKSI, EORTCQLQ30, and GP5 — showed a temporary dip in global and physical/role function during therapy in the LITESPARK-022 pembrolizumab/belzutifan arm, yet this dip did not reach predefined clinically meaningful levels despite more than 50% grade 3–4 toxicity in that arm. Separately, quality-of-life data for lenvatinib plus belzutifan versus cabozantinib in advanced renal cancer (LITESPARK-011) showed little difference between the two arms. Discussants contend that CTCAE captures acute, reversible toxicity but may not reflect the toxicity that matters most to patients or predict long-term quality-of-life impact, as seen with adjuvant checkpoint inhibition at ASCO26.

The EAU 2026 prostate cancer guideline updates include early salvage radiotherapy for biochemical recurrence, PSMA PET as the preferred imaging modality, and the position that ADT alone is no longer sufficient for most mHSPC patients, with prostate radiotherapy retained as important in low-volume metastatic disease. Working Group 4 has replaced the terms castration-sensitive and castration-resistant with androgen pathway modulator naive, sensitive, and resistant — a shift applicable to any treatment, not just ADT. The practical challenge is how quickly community practice can adopt both the imaging upgrade and the revised nomenclature.

In cisplatin-ineligible MIBC (n=344), enfortumab vedotin plus pembrolizumab produced a pCR of 57.1% versus 8.6%, 2-year EFS of 74.7% versus 39.4% (HR 0.40), and 2-year OS of 79.7% versus 63.1% (HR 0.50), establishing this regimen as the perioperative standard in this population. Clinicians framing the same week's data note that the ADC/immunotherapy combination is replacing neoadjuvant chemotherapy while reaching over 50% pathologic complete response. With both cisplatin-eligible and cisplatin-ineligible populations now covered by perioperative EV plus pembrolizumab data, the remaining role of surgery alone is under active scrutiny.

The phase III FLAME trial demonstrated that isotolerated focal boosting of intraprostatic tumors improves biochemical progression-free survival without significantly increasing side effects, whereas whole-gland dose escalation can improve biochemical progression-free survival but may increase toxicity. ASCENDE-RT 15-year data compared external beam dose escalation versus brachytherapy boost in approximately 400 patients receiving pelvic radiation and one year of hormone therapy, providing a longer-term reference point for the boost debate. The central question is whether focal dose escalation to the dominant intraprostatic lesion should replace whole-gland dose escalation as the default radiotherapy approach.

A KCRS26 presentation highlighted recurrent genomic alterations associated with RCC brain metastases, and the same group built RCC-BM, a tool predicting brain metastasis outcomes from patient-level parameters. Discussants questioned whether an approximately 60-month OS gap attributed to screening MRI could be largely driven by immortal time or lead-time bias, noting that attributing such a survival benefit solely to screening MRI is difficult to justify without prospective data. The debate centres on whether asymptomatic brain MRI screening should be routine in metastatic RCC.

Ultrasensitive whole-genome-informed ctDNA testing in clear cell renal cancer appears more sensitive than exome-based ctDNA analysis; at approximately 1 ppm, metastatic detection was 100% (32/32), adjuvant/surveillance NPV was 100% with 5/8 ctDNA-positive patients progressing, and ctDNA rise preceded radiographic progression by approximately 6.8 months. In KEYNOTE-564, baseline ctDNA positivity occurred in only 5–10% of patients but with over 95% specificity, and ctDNA-positive patients had a 20% absolute 2-year recurrence risk reduction with adjuvant pembrolizumab. The question for trial design is whether an approximately 6.8-month lead time over imaging is sufficient to justify ctDNA-guided adjuvant studies in this setting.

A 12-week telehealth exercise program improved HRQOL by 9 points, reduced fatigue by 21 points, and improved iRAE scores by 10 points in patients with metastatic RCC on checkpoint inhibitors with or without TKIs. A related research letter reported that home-based exercise improved HRQOL and symptoms in men with advanced prostate cancer on systemic therapy, with greater benefit at moderate-high adherence. Across metastatic GU cancers, participation in a remote exercise program was associated with increased quality of life and decreased fatigue, prompting the question of whether such programs belong in standard supportive care.

In low serum conditions, no growth factor rescued clear cell RCC proliferation — only oleate did, roughly 6-fold — indicating these tumors are lipid-limited. Clear cell RCC preferentially imports exogenous lipids to build new membranes rather than synthesising their own, and knocking out fatty acid synthase barely slows them. The keynote linked obesity, immune metabolism, and RCC as deeply interconnected pathways that may reveal new therapeutic targets, with discussants considering whether lipid peroxidation and ferroptosis could be turned into a therapeutic vulnerability.

In 152 patients followed for a median of 111 months after salvage lymph node dissection, 59% developed distant metastatic disease and 34.5% experienced locoregional nodal recurrence; the number of positive lymph nodes independently predicted metastatic progression, increasing risk by 14% per positive node. Metachronous oligorecurrent hormone-sensitive prostate cancer is identified as the population where metastasis-directed therapy alone is most supported, while systemic therapy should not be omitted in patients with PSA doubling time under three months or high-risk mutations including BRCA and TP53. The persistent distant metastasis rate of 59% sharpens the debate over which nodal-recurrent patients should be offered surgery versus systemic intensification.

Keynote data published in Nature and Nature Medicine were presented as convincing evidence for an impact of diet on therapeutic outcomes in kidney cancer, with discussants asking whether Lactobacillus johnsonii or an obesogenic diet could be a key mediator. Against this backdrop, fecal microbiota transplantation combined with checkpoint inhibitors produced no objective responses in 12 ICI-refractory patients, with 5/12 recording stable disease, a median PFS of 1.5 months, and a median OS of 10.1 months. Discussants frame the next frontier as how to safely and reproducibly manipulate the microbiome — determining not only which bacteria matter but how to translate microbiome biology into a treatment for kidney cancer.

PROTEUS randomized 2,109 patients with high-risk localized or locally advanced prostate cancer to perioperative apalutamide plus ADT versus placebo plus ADT before radical prostatectomy; both dual primary endpoints were met, with pathologic complete response/minimal residual disease in 8.9% versus 1% and metastasis-free survival HR 0.80, while event-free survival HR was 0.71. However, because PSMA PET imaging was not integrated in the study from the start, the PET-based MFS endpoint is available only for a subset of patients, and not every positive PET scan counted under the FDA-aligned definition. Adding apalutamide to standard hormone therapy before and after surgery was described as significantly improving outcomes for men with high-risk disease, yet the endpoint debate raises the question of whether MFS remains an interpretable primary endpoint for perioperative prostate cancer trials in an era of widespread PSMA PET availability.

FGFR3 alterations were found in 17% of urothelial carcinomas, with S249C and Y373C accounting for most mutations; 76% of FGFR3-altered tumors exhibited variant histology, most commonly micropapillary (39%) and squamous differentiation (21%). A multidisciplinary European review recommends FGFR3 testing at diagnosis using validated PCR or next-generation sequencing to enable timely access to erdafitinib, with liquid biopsy considered a complementary approach pending further evidence. While morphology cannot replace molecular testing, recognising these histologic patterns may help identify patients most likely to benefit from FGFR3 testing and targeted therapy in advanced urothelial carcinoma, with micropapillary or squamous morphology now discussed as a potential trigger for reflex testing.

The FDA granted priority review to talazoparib plus enzalutamide for HRR-altered metastatic castration-sensitive prostate cancer, with a decision expected in Q4 2026, supported by interim TALAPRO-3 results showing the combination significantly delayed disease progression versus enzalutamide alone. TALAPRO-3 was a 599-patient phase 3 trial randomizing HRR-mutated metastatic hormone-sensitive prostate cancer patients to ADT plus enzalutamide plus talazoparib versus ADT plus enzalutamide plus placebo. Clinicians raise concerns about earlier and earlier PARP inhibitor exposure and the risk of long-term toxicity when patients are treated for years in mHSPC, and the central trade-off is weighing years-long PARP inhibitor exposure against the benefit of delayed progression.

Atezolizumab was approved for adjuvant treatment of muscle-invasive bladder cancer with ctDNA minimal residual disease after cystectomy, with IMvigor011 showing improved DFS and OS in MRD-positive patients. Commentators describe the approval as moving precision oncology earlier, with ctDNA MRD testing after cystectomy now central to guiding adjuvant therapy decisions. Post-surgical bladder cancer care is becoming more personalised, and the operative question is which patients after cystectomy should be tested and how frequently.

SWOG S1600, a multicenter phase 3 RCT, found no reduction in 30- or 90-day complications with specialized immunonutrition versus standard nutrition after cystectomy, while exploratory DFS and OS data favored immunonutrition. Separately, clinicians stress that negative urethral washings or imaging after cystectomy do not always confirm a clear urethra, and that urothelial cancer can persist even with negative margins, with patient-reported spotting or leakage deserving closer evaluation. The debate is whether the exploratory survival signals from SWOG S1600 justify a dedicated oncologic-endpoint trial of immunonutrition after cystectomy.

In a multi-institution study of 217 patients at 7 centers undergoing deferred consolidative nephrectomy after ICI-based therapy for advanced RCC, pathologic complete response with 0% residual viable tumor occurred in approximately 13% of patients and tracked with favorable 18-month PFS; notably, half of pCR patients never resumed ICI and still did well. Clinicians debate whether immunotherapy should continue after a patient achieves pCR following consolidative nephrectomy, acknowledging that pCR is linked to favorable outcomes but that it remains unknown whether further treatment prevents relapse or only adds toxicity. The central question is whether stopping checkpoint inhibitor therapy after pCR at consolidative nephrectomy is defensible outside a formal trial.

Clonal hematopoiesis was detected in 66% of patients receiving 177Lu-PSMA-617 and was associated with a trend toward increased hematologic toxicity, although overall survival was unaffected; serial analyses revealed frequent emergence and expansion of DNA damage repair-related clonal hematopoiesis clones during treatment. A separate report describes a ctDNA blood test that identifies mCRPC patients most likely to benefit from radium-223 radiopharmaceutical therapy. The clinical question is whether patients should be screened for clonal hematopoiesis before starting 177Lu-PSMA-617 in order to anticipate hematologic toxicity.

In the COSMIC-313 analysis, higher baseline KIM-1 was associated with poor-risk IMDC disease and worse overall survival, positioning KIM-1 as a blood biomarker that could help guide treatment decisions in kidney cancer. The data were presented at KCRS26 alongside discussion of why collaborative multicentre analyses are important for advancing biomarker research in RCC. The question that follows is whether dynamic changes in KIM-1 during treatment could serve as an early marker of treatment response.

ICIs achieved pCR 61.9% vs 3.7% with FLOT and better downstaging; survival similar.
👉 ICIs show superior pathologic response—supporting organ-sparing approaches.
GU Oncology
✅Metastasis-Directed RT Without Systemic Therapy in RCC (Lancet Oncology) #KidneyCancer
In oligometastatic ccRCC, MDT achieved PFS 17.7 mo and systemic therapy-free survival 34 mo with low toxicity.
👉 MDT alone can delay systemic therapy with durable control.
✅Salvage Brachytherapy for Locally Recurrent #ProstateCancer (Green Journal / ESTRO)
Largest cohort (n=266): median bPFS 40 mo; high-risk disease, HDR, focal BT, and ADT linked to worse outcomes.
👉 Salvage BT offers feasible control with acceptable late toxicity.
👩⚕️ Gyn Oncology
✅Node Boost in Stage IIIC #CervicalCancer (IJGC)
Radiotherapy boost to positive nodes improved OS (107 vs 91 mo), especially in patients with smaller/fewer nodes.
👉 Nodal boost improves survival in selected patients.
Reirradiation Across Tumor Types (CA: A Cancer Journal for Clinicians)
#RadOnc ☢️
✅Advances in IMRT/SBRT make reRT viable but high-risk—requires careful selection, imaging, and multidisciplinary input.
👉 Reirradiation offers opportunity for control with thoughtful planning.
And much more!
