FDA approved tucatinib plus trastuzumab/pertuzumab as maintenance for HER2-positive metastatic breast cancer (HER2CLIMB-05, median PFS 24.9 vs 16.3 months, HR 0.64), NRG-BR002 showed metastasis-directed ablation does not improve PFS or OS in oligometastatic breast cancer, and NEJM published a broad review of systemic breast cancer therapy.

NRG-BR002: A Randomized Phase II/III Trial of Metastasis-Directed Ablation With First-Line Systemic Therapy for Oligometastatic Breast Cancer. β Chmura Steven J et al., J Clin Oncol
Of 129 patients enrolled, 125 were eligible for analysis. Each had β€4 metastases and no progression on β€12 months of first-line systemic therapy. Median PFS was 23.0 months without ablation versus 19.5 months with ablation (HR 0.92; 95% CI, 0.57 to 1.47; one-sided P = .36). OS did not differ (HR 1.07; 95% CI, 0.60 to 1.89). Median age was 54 years, 79% had HR-positive/HER2-negative disease, and 93% received ablation by SBRT. No grade 5 toxicities occurred. An exploratory analysis suggested improved PFS with ablation in patients with low or absent pretreatment CTCs.
Clinical bottom line: Metastasis-directed ablation added to first-line systemic therapy should not be offered routinely in oligometastatic breast cancer outside a trial or for palliation. Any future use should be biomarker-selected, for example by low or absent circulating tumor cells.
Advances in Systemic Treatment of Breast Cancer. β Hurvitz Sara Alsterlind et al., N Engl J Med
Since 2020, systemic therapy has expanded across biologic subtypes through targeted therapies, antibody-drug conjugates and immunotherapy. In HR-positive/HER2-negative disease, CDK4/6 inhibitors remain foundational, and PI3K pathway inhibitors, oral SERDs and ADCs have transformed management after endocrine resistance. Trastuzumab deruxtecan is moving into earlier lines in HER2-positive disease, and checkpoint inhibitors have improved outcomes in metastatic and early-stage triple-negative disease. The review names the remaining challenges as sequencing, predictive biomarkers, ctDNA integration and equitable access.
Clinical bottom line: The review changes no single treatment decision. It is the reference oncologists will cite for how biomarker-guided systemic therapy, ADCs and immunotherapy are now organized by subtype across the breast cancer continuum.
Right-sizing treatment for HER2-positive breast cancer: evolving algorithms across the disease continuum. β Tarantino Paolo et al., Nat Rev Clin Oncol
HER2-positive breast cancer is now highly curable in early-stage disease, and a substantial proportion of metastatic patients survive beyond 5 years. Trastuzumab deruxtecan has moved into first-line metastatic, adjuvant residual-disease and neoadjuvant settings. This shift leaves gaps in evidence on sequencing, retreatment and patient selection. In advanced disease, the authors call for optimizing maintenance with palbociclib or tucatinib after induction and integrating it with first-line trastuzumab deruxtecan plus pertuzumab. Tissue, plasma and imaging biomarkers are beginning to guide these decisions.
Clinical bottom line: This review gives clinicians a practical framework for de-intensifying low-risk early HER2-positive disease and for choosing among maintenance options after induction. This week's tucatinib maintenance approval makes that maintenance choice newly urgent.
Impact of Primary Prophylactic Colony-Stimulating Factor Delivery Method on Febrile Neutropenia: Secondary Analysis of a Pragmatic Cluster Randomized Clinical Trial. β Shih Lauren et al., JCO Oncol Pract
This secondary analysis of SWOG S1415CD included 1,713 patients with a median age of 55 years. Prophylactic CSF was delivered in clinic in 773 (45%), by on-body device in 862 (50%) and by self-injection in 78 (5%). Febrile neutropenia incidence was 3.8%, 5.0% and 6.4% respectively (P = .29). FN was associated with high-risk FN regimens (OR 2.97; P = .04) and Zubrod performance status β₯2 (OR 4.99; P = .01), but not with delivery method or agent.
Clinical bottom line: Febrile neutropenia rates were equally low however prophylactic growth factor was delivered. Clinicians can therefore choose the delivery method on cost, convenience and access rather than presumed efficacy.
Efficacy of Cryotherapy in Preventing Taxane-Induced Neuropathy: The CryoPac Randomised Controlled Trial. β Lendorf M E et al., Ann Oncol
Of 268 randomised patients, 217 were analyzed (cryotherapy 123, usual care 94). CIPN incidence at end of treatment was 74% (91/123) with cryotherapy versus 84% (79/94) with usual care (P= .071), so the primary endpoint was not met. Patient-reported subscales favored cryotherapy for tingling in hands (P=.025), numbness in hands (P=.025) and feet (p=0.0005), and trouble manipulating small objects (P=.006). There was no difference in CTCAE grade β₯2 CIPN (P = .2). Compliance with cryotherapy was 67%.
Clinical bottom line: Frozen gloves and socks failed the primary endpoint and should not be presented as preventing taxane neuropathy. The consistent reduction in patient-reported sensory symptoms still makes cryotherapy a reasonable optional measure for motivated patients.

Sara Hurvitz's NEJM review was shared as a state-of-the-art reference for metastatic breast cancer treatment. Clinicians noted that the approvals since 2020 needed a two-page centerfold to display, and that PATINA made the list of advances. The review covers CDK4/6 inhibitors as foundational in HR-positive/HER2-negative disease, trastuzumab deruxtecan moving into earlier lines, and checkpoint inhibitors in triple-negative disease. It also names sequencing, predictive biomarkers, ctDNA integration and access as unresolved, which leaves open which agents are truly standard of care and which cannot yet be sequenced rationally.

On October 7, 2026, the FDA approved tucatinib with trastuzumab and pertuzumab as maintenance after induction, based on HER2CLIMB-05. In that trial, PFS improved over placebo, OS data were immature, and hepatotoxicity carries a boxed warning. The label does not specify the type of induction, and no sequencing trial exists. Some clinicians would not make tucatinib their preferred maintenance given DB-09 and PATINA, and in one poll about two thirds would continue T-DXd or use HP alone after T-DXd/pertuzumab. The open question is whether HR status or brain-metastasis risk should decide between tucatinib plus HP, PATINA-style palbociclib, or continued T-DXd.

Tarantino and colleagues argue that the central challenge is now right-sizing therapy across the HER2-positive continuum. In early-stage disease, that means de-intensifying low-risk tumors and escalating for residual disease. In advanced disease, it means fitting palbociclib or tucatinib maintenance alongside first-line T-DXd plus pertuzumab, whose optimal duration and sequencing remain uncertain. On-treatment MRI can already inform neoadjuvant chemotherapy, while HER2DX, MRD and quantitative HER2 are still in validation. That leaves open which early-stage patients can safely be de-intensified, and which biomarker is ready to make that call outside a trial.

Adding ablation, mostly SBRT, to systemic therapy did not improve PFS or OS, and the trial did not advance to phase 3. There was no grade 5 toxicity. Investigators reported in-field progression of 33% per patient and asked for per-lesion data. An exploratory signal favored ablation in patients with low or undetectable CTCs. Clinicians debate whether any patient should still receive consolidative SBRT off-trial, or whether selection must now rest on biomarkers rather than on counting metastases.

Alliance Foundation Trials and Natera launched AFT-70 NAVIGATE, a global Phase III trial in ER+/HER2- early breast cancer. It pairs giredestrant with Signatera MRD monitoring and adds a CDK4/6 inhibitor only if MRD turns positive, across 2,000+ patients at ~200 sites. No efficacy results have been reported yet. The open question is whether MRD positivity is a safe trigger for escalation, or whether withholding adjuvant CDK4/6 inhibition from monarchE/NATALEE-eligible patients risks undertreating them.

In 272 patients with HER2-non-amplified metastatic breast cancer treated with T-DXd, activating ERBB2 mutations were associated with longer PFS: 11.0 vs 6.2 months (adjusted HR 0.49). The signal also appeared in HER2 IHC 0 tumors, and in preclinical models the mutations enhanced T-DXd internalization and cytotoxicity. Current clinical assays do not capture internalization or payload delivery. That leaves open whether ERBB2 mutation status, rather than IHC score, should decide who receives T-DXd.

A new JCO OP review reports that low-dose vaginal estrogen keeps serum estradiol in the postmenopausal range. Observational data show no increase in recurrence or mortality, including in ER+ disease and on tamoxifen. Because the safety data are only observational, clinicians question whether it should now be offered routinely for genitourinary symptoms in survivors on endocrine therapy.

The 2nd Florence International Consensus, endorsed by ESTRO, addressed when to pause treatment, when to combine, and how to reduce adverse events. A dedicated session covered timing, toxicity signals and practical recommendations for combining CDK4/6 inhibitors with radiotherapy. The panel argued that locoregional control matters more as systemic therapies control distant disease, and that sequencing depends on toxicities that may be additive or synergistic. The unresolved question is whether adjuvant CDK4/6 inhibitors should be held during radiotherapy or given concurrently.

Updated HER2CLIMB data showed that adding tucatinib to capecitabine/trastuzumab improved overall survival and intracranial PFS in patients with HER2+ brain metastases. The final analysis also showed an overall survival benefit for the tucatinib combination. One clinician framed brain metastases as the hard part of sequencing: if T-DXd is used up front, tucatinib is what remains. The debate is whether to use tucatinib early in maintenance or reserve it for CNS progression.
