Giredestrant plus Everolimus in Advanced Breast Cancer. โ€” Mayer Erica L et al., N Engl J Med

In this phase 3 open-label trial, 373 patients with ER-positive, HER2-negative advanced breast cancer progressing after a CDK4/6 inhibitor plus endocrine therapy were randomized to giredestrant-everolimus (183) or standard therapy-everolimus (190). Among 207 patients with ESR1-mutated tumors, median progression-free survival was 10.0 vs 5.5 months (HR 0.38; 95% CI, 0.27 to 0.54; P<0.001). In the overall population it was 8.8 vs 5.5 months (HR 0.56; 95% CI, 0.44 to 0.71; P<0.001). Adverse events occurred in 98.9% vs 96.8% of patients, most commonly stomatitis (47.3% vs 48.9%), diarrhea (26.9% vs 22.6%), and anemia (23.6% vs 21.0%).

Clinical bottom line: After progression on a CDK4/6 inhibitor in HR+/HER2- advanced breast cancer, the all-oral giredestrant-everolimus doublet belongs primarily in ESR1-mutated disease, where PFS roughly doubles, not in all comers.

Hypofractionated versus conventional fractionated postmastectomy radiotherapy for patients with high-risk breast cancer (CHN HYPOPMRT): 10-year outcomes from a randomised, non-inferiority, open-label, phase 3 trial. โ€” Sun Guang-Yi et al., Lancet Oncol

The trial randomized 820 patients to 50 Gy in 25 fractions over 5 weeks or 43.5 Gy in 15 fractions over 3 weeks to the chest wall and supraclavicular region; 810 were analysed. Median follow-up was 11.5 years, median age was 49 years, and 761 (94%) had stage III disease. At 10 years, cumulative locoregional recurrence was 10.3% with conventional vs 12.0% with hypofractionated radiotherapy (HR 1.19, 95% CI 0.79-1.82), with no grade 4-5 events, brachial plexopathy, serious adverse events, or treatment-related deaths.

Clinical bottom line: Three-week hypofractionated postmastectomy radiotherapy with regional nodal irradiation holds up at 10 years in high-risk, mostly stage III disease, and conventional 5-week fractionation no longer needs to be the default.

Palliative Care for Advanced Cancer in Medicare Advantage and Physician Networks. โ€” Hu Xin et al., JAMA Netw Open

This retrospective SEER-Medicare cohort included 135 402 beneficiaries aged 66 or older with distant-stage breast, colorectal, lung, pancreatic, or prostate cancer; 32.3% were in Medicare Advantage and 67.7% in traditional Medicare. Medicare Advantage beneficiaries had higher 6-month cumulative incidence of palliative care (13.3% vs 9.3%; AHR 1.39, 95% CI 1.35-1.44; P<.001), but after matching on treating oncologist there was no difference (AHR 1.05, 95% CI 0.98-1.12; P=.19). Medicare Advantage beneficiaries had lower systemic therapy receipt (-5.38 percentage points) and higher hospice enrollment (3.42 pp), differences that persisted after matching.

Clinical bottom line: Higher palliative care use in Medicare Advantage is largely explained by which oncologists patients see, whereas lower systemic therapy and higher hospice use persist regardless of physician network โ€” a coverage-level difference oncologists should factor into advanced breast cancer care planning.

How these are chosen: filtered for clinical relevance, grouped by the new development discussed, then ordered by engagement (bookmarks and reposts).

evERA published in NEJM: giredestrant plus everolimus doubles PFS after CDK4/6 inhibition, but the benefit sits in ESR1-mutated disease

With evERA now in print, clinicians focused on where the PFS gain sits: it was concentrated in ESR1-mutated tumors, while in tumors without a detectable ESR1 mutation PFS was nearly identical between arms. Overall survival remains immature but shows a favorable numerical trend (ITT HR 0.69; ESR1-mutated HR 0.62), and some questioned the comparator given that 31% of patients had PIK3CA mutations, asking whether alpelisib + fulvestrant would better clarify the regimen's role. In a clinician poll on second-line choice for an ESR1-mutant patient, imlunestrant + abemaciclib (43.1%) edged out giredestrant + everolimus (32.5%). The open question is which doublet to choose after CDK4/6 inhibitor progression with an emergent ESR1 mutation, and whether either has a place in ESR1 wild-type disease.

Key numbers: ESR1-mut PFS 10.0 vs 5.5 mo, HR 0.38 ยท Overall PFS 8.8 vs 5.5 mo, HR 0.56 ยท ESR1 wild-type PFS 5.7 vs 5.5 mo, HR 0.84 ยท ORR 23.8% vs 11.7% ยท ESR1-mut ORR 26.6% vs 13.8%

Blood-derived neoantigen identification could remove the tumor biopsy requirement for personalized cancer vaccines

Personalized neoantigen vaccines have improved outcomes in refractory malignant melanoma and in pilot studies including triple negative breast cancer, but require a tumor biopsy to identify antigens; a new study shows these can be obtained from a blood sample. Separately, serial cfDNA testing in more than 12,000 patients with metastatic breast cancer detected newly emerging resistance mutations before clinical progression, and a liquid biopsy tumor board debated whether ctDNA can move from predicting recurrence to guiding treatment in early breast cancer. The debate is which decision blood-only profiling should drive first: vaccine manufacture, or a change of systemic therapy before radiographic progression.

A JCI review argues breast cancer immunotherapy selection should measure the patient's immune status, not just the tumor

A review in the Journal of Clinical Investigation on optimal use of immunotherapy in breast cancer emphasizes the patient's immune status, not just the tumor's. A multi-cohort study in advanced triple-negative breast cancer identified a transcriptomic signature that stratifies progression-free survival after chemo-immunotherapy: immune-enriched tumors with organized lymphoid architecture had better outcomes, while fibrotic and angiogenic microenvironments, often with PI3K alterations, had poorer outcomes. The question is whether selection in advanced TNBC should move beyond PD-L1 to microenvironment signatures and host immune status, and who would pay for that testing today.

Gut microbiome patterns track with early-onset breast and colon cancer, treatment toxicity and survival

Gut microbiome patterns were linked to early-onset breast and colon cancer, adverse effects of treatment, and survival prediction. The Lancet Oncology's October issue put cancer and the microbiome on its cover. The open question is whether microbiome profiling is close enough to prognostic or toxicity-predictive use in early-onset breast cancer to justify collecting stool samples on trial, or whether it remains descriptive.
