Safety and Efficacy of Ceralasertib plus Olaparib for Advanced/Metastatic Triple-Negative Breast Cancer in Three Molecular Strata: The Phase II VIOLETTE Study. โ€” Tutt Andrew et al., Clin Cancer Res

VIOLETTE (NCT03330847) randomized 273 patients 1:1:1 to olaparib, ceralasertib plus olaparib, or adavosertib plus olaparib as second-/third-line treatment across three molecular strata of PARP inhibitor-naive advanced TNBC; 114 received olaparib, 112 ceralasertib plus olaparib, and 47 adavosertib plus olaparib, with the adavosertib arm terminated early due to unacceptable toxicity. Median PFS with ceralasertib plus olaparib was 7.4, 3.9, and 3.6 months in the BRCA1/2m, non-BRCA1/2m HRRm, and non-HRRm strata, respectively, and did not differ significantly from olaparib (HR 1.02 [90% CI, 0.63โ€“1.66], 0.54 [90% CI, 0.28โ€“1.03], and 0.76 [90% CI, 0.50โ€“1.14], respectively). In the non-HRRm group, ORR was significantly improved with ceralasertib plus olaparib (15.4%) versus olaparib (3.9%; OR, 4.45; 90% CI, 1.30โ€“21.20; P = 0.0425), with no unexpected safety signals reported for either agent.

Clinical bottom line: Adding the ATR inhibitor ceralasertib to olaparib does not prolong PFS in any molecular stratum of PARP inhibitor-naive advanced TNBC, so despite a higher response rate in non-HRR-mutant disease this combination should not move into practice outside trials.

ESMO 2026 late-breaking abstracts drop, and breast oncologists publish competing top-10 lists

The ESMO 2026 late-breaking abstracts are out, with multiple trials in breast oncology to be presented in Madrid. Clinicians circulated competing top-10 breast cancer abstract lists for ESMO 2026, with at least one sorted by day, session, and a "why it matters" context for the field. The central debate: which late-breaking breast abstract is most likely to change practice on the day it is presented.

DBCG Skagen Trial 1 prospectively validates the ESTRO target volume consensus guideline in 2,963 patients

The phase III DBCG Skagen Trial 1, enrolling 2,963 breast cancer patients, provided prospective validation of the ESTRO target volume guideline; loco-regional recurrences occurred almost exclusively within the ESTRO CTVs, with no evidence supporting routine expansion of ESTRO-defined volumes. Separately, the 5-year analysis of the SUPREMO QOL substudy found that chest wall radiotherapy leads to a modest increase in local chest wall symptoms that improve over time, without significant effect on overall QOL and other domains. The question for radiation oncologists is whether any remaining justification exists for expanding target volumes beyond consensus contours given these convergent findings.

Dana-Farber's updated consensus statement on first-line treatment for metastatic triple-negative breast cancer

Dana-Farber released an updated consensus statement on first-line treatment for metastatic TNBC, prompting clinicians to articulate a broader concern: the drive to move every successful metastatic breast cancer therapy to the front line is stifling innovation, compounded by a "me too" mentality in drug development that crowds the same targets. Specialists argue that TNBC patients need effective treatment options after a TROP2-directed topoisomerase-1 ADC, noting that ADCs are "better chemo" but do not transform the tail of the OS curve. The unresolved question is what the evidence-based second-line option is after a TROP2-directed ADC, given the current absence of new targets and payloads.

NCI's BRIDGE AI tool predicts which breast cancers will achieve pathologic complete response to neoadjuvant chemotherapy

NCI-supported researchers developed an AI tool called BRIDGE that can predict which breast cancers will fully respond to chemotherapy given before surgery. Alongside this development, clinicians drew a distinction between the complementary roles of pCR and residual cancer burden: pCR reports response, RCB refines prognosis, and not all non-pCR outcomes are equivalent โ€” RCB guides escalation decisions. The open question is whether a pre-treatment pCR prediction should be used to de-escalate neoadjuvant chemotherapy or only to select patients for escalation once residual cancer burden is known.

The TOP1-payload ADC loop: every new breast ADC trial excludes patients pretreated with a TOP1 ADC

A pointed observation circulated this week: multiple ADCs with variable targets showing high efficacy in metastatic breast cancer share two consistent features โ€” a TOP1 payload and only a handful or no TOP1-ADC pretreated patients enrolled in the trial. A recent review on antibody-drug conjugates in breast cancer in the first-line and curative setting was shared alongside the debate, and ADC sequencing and whether ADC target expression matters were flagged as unresolved topics at dedicated sessions. The structural question is whether regulators and cooperative groups should require new TOP1-payload ADC trials to enroll patients previously treated with a TOP1 ADC before those agents are approved.

A JCO piece on carboplatin dosing reopens the case for carboplatin-sparing regimens in HER2-positive early breast cancer

A JCO piece arguing that adequate calculation of carboplatin dosage remains controversial after decades of usage added weight to the case for carboplatin-sparing approaches in HER2-positive early breast cancer, where recent data suggest a marginal therapeutic role for the drug. In the phase III Neo-Healer trial, anbenitamab plus HB1801 chemotherapy improved pathologic complete response across key subgroups of ERBB2-positive breast cancer โ€” including HR-positive and HR-negative disease โ€” with or without carboplatin, adding to the debate. Clinicians are now asking whether uncertain AUC-based dosing and marginal incremental benefit justify dropping carboplatin from neoadjuvant HER2-positive regimens outright rather than continuing to dose by convention.

IMpassion050 final analysis: atezolizumab adds nothing to neoadjuvant HER2-directed therapy, with an unexplained DFS signal in PD-L1-negative disease

The final IMpassion050 analysis, at approximately 44 months of follow-up, confirmed that adding atezolizumab to HER2-directed therapy did not significantly improve EFS (HR 0.90) or DFS (HR 0.71) in high-risk HER2-positive early breast cancer; three-year EFS was 91.4% versus 89.0% (HR 0.90; 95% CI, 0.50โ€“1.59). A DFS signal in PD-L1-negative disease (HR 0.44) attracted attention but is based on few events and wide confidence intervals. Clinicians are divided on whether this signal justifies any further checkpoint inhibitor testing in HER2-positive early breast cancer, or whether the hypothesis is effectively closed given the event counts.

SERENA-6 โ€” my running notes ๐Ÿ“

โ€ข Who & when to test? After โ‰ฅ6 months of 1L AI + CDK4/6i, serial ctDNA monitoring can detect emerging ESR1m before radiological progression.

โ€ข Does switching early matter? AI โ†’ camizestrant while continuing the same CDK4/6i improved PFS: 9.2 โ†’ 16.8 months | HR 0.45

โ€ข Does the benefit persist? PFS2: 19.1 โ†’ 25.7 months | HR 0.63, with chemotherapy/ADC-free survival also prolonged.

โ€ข Co-mutations? PFS benefit was preserved with PIK3CA and TP53.

โ€ข What happens biologically? ESR1 mutant allele frequency fell profoundly within weeks after switching to camizestrant.

โ€ข Which CDK4/6i? SERENA-6 included palbociclib, ribociclib and abemaciclibโ€”the same CDK4/6i was continued.

โ€ข The bigger question: Do we always wait for the scan to progressโ€”or can we detect endocrine resistance molecularly and intercept it earlier?
