OPTIMA/TAILORx/RxPONDER gene-expression debates, final FeDeriCa subcutaneous HER2 data, SUPREMO 5-year quality-of-life results and HORIZON-Breast01's 30.6-month PFS dominated breast cancer discussion this week.

Investigation of a prostate cancer radiation response signature in the breast cancer SweBCG91RT randomized trial. β Zhao Shuang G et al., Clin Cancer Res
PORTOS scores were calculated from gene expression profiles of 765 tumor samples from SweBCG91RT, a randomized trial of adjuvant whole-breast RT versus observation following breast-conserving surgery in node-negative stage I-IIA breast cancer. A significant interaction (P = 0.049) between PORTOS and RT was observed for 10-year cumulative incidence of any recurrence; patients in the top 75% of PORTOS scores derived substantial benefit from RT (SHR = 0.46, P < 0.001), compared to those in the bottom 25% (SHR = 0.73, P = 0.24). Higher PORTOS scores were also associated with increased risk of RT-related breast pain, though the interaction P-value was not significant (interaction P = 0.07).
Clinical bottom line: A 24-gene radiation response signature borrowed from prostate cancer predicts both radiotherapy benefit and radiation-related breast pain in node-negative early breast cancer, and adds information beyond ARTIC and POLAR.
How these are chosen: filtered for clinical relevance, grouped by the new development discussed, then ordered by engagement (bookmarks and reposts).
With 6 FDA approvals in breast cancer already in 2026, treatment algorithms covering early stage and metastatic breast cancer by subtype required updating β with the possibility of another revision before year-end. Among the approvals driving this pace is vepdegestrant, the first PROTAC protein degrader approved for ER+/HER2- advanced breast cancer; in ESR1-mutant disease it doubled PFS versus fulvestrant (5 vs 2.1 months). The central question for practices is how to keep sequencing decisions coherent when six approvals land within a single calendar year.

RxPONDER, TAILORx, and OPTIMA have been circulated together in AI-generated comparisons among breast oncologists, yet practical adoption of OPTIMA remains uncertain β particularly the choice between OncotypeDx and PAM50 scoring and identifying the right patient. TAILORx data clarify one boundary: in HR+/HER2-, node-negative disease with RS 21β25, endocrine therapy alone matched chemoendocrine therapy in women over 50, while women under 50 with that same midrange score showed a small chemotherapy benefit. A diagnostic test co-developed by Charles Perou has separately been reported to accurately predict whether breast cancer patients need chemotherapy. The unresolved debate is which test should drive the chemotherapy decision in node-positive HR+/HER2- disease when OPTIMA's PAM50-based selection and the Recurrence Score point in different directions.

A new paper in JNCI analyzed Recurrence Score gene group components and outcomes in the RxPONDER trial (SWOG S1007), prompting debate over how pN1 disease should be stratified. Commentators argue pN1 should be split into high risk (G3 or T3) versus non-high risk (G1-2 and T1-2 with low RS): in the non-high-risk group, where chemotherapy is omitted, the 5-year risk of distant metastases is <7β10% and the additional CDK4/6 inhibitor benefit is less than 3% at 5 years. Premenopausal patients with N1 tumors were identified as the group where these data are most consequential. The open question is whether pN1 HR+/HER2- disease should be formally subdivided by grade and tumor size before a chemotherapy or adjuvant CDK4/6 inhibitor decision is made.

A new review in ESMO Open addresses HER2DX and its role in HER2-positive early breast cancer, appearing alongside a JNCCN review describing a clear shift toward personalized neoadjuvant treatment. That JNCCN review identifies biomarkers β HER2DX, ctDNA, PET response, and molecular subtype β as candidates to guide treatment de-escalation in this setting. The central question for clinicians is whether HER2DX is mature enough to select patients for neoadjuvant chemotherapy de-escalation outside of a trial.

After 10 years of follow-up, chest wall RT offers no survival benefit in intermediate-risk breast cancer β except a small drop in local recurrence β and the triple-negative subgroup showed worse OS with RT (HR 1.91). The 5-year SUPREMO quality-of-life substudy found that chest wall radiotherapy leads to a modest increase in local chest wall symptoms, which improve over time, without significant effect on overall quality of life or other domains. Patients receiving chemotherapy or undergoing sentinel node biopsy followed by axillary clearance experienced less favourable symptom outcomes. If radiotherapy after mastectomy neither improves survival nor materially worsens global quality of life, clinicians are left to define on what basis it should still be offered in intermediate-risk disease.

A Lancet Oncology piece argues that in TNBC brain metastases the blood-brain barrier is no longer merely a barrier to drug delivery but an actionable therapeutic target. A phase II trial in breast cancer leptomeningeal disease is enrolling, evaluating intrathecal cDC1s combined with intrathecal trastuzumab (HER2+) or intrathecal nivolumab (HER2-). Patient-reported outcomes from the TBCRC 022 study of neratinib and ado-trastuzumab emtansine for HER2-positive breast cancer brain metastases were also published in The Breast. The question for trial designers is whether treating the blood-brain barrier as a druggable target should change how CNS-directed trials in breast cancer are structured.

π° Trastuzumab rezetecan versus pyrotinib plus capecitabine for patients with HER2-positive metastatic breast cancer (HORIZON-Breast01): interim analysis of a multicentre, open-label, randomised, controlled, phase 3 trial
In this phase 3 trial of patients with 𧬠HER2-positive unresectable or metastatic #BreastCancer previously treated with trastuzumab and taxane therapy, trastuzumab rezetecan significantly outperformed pyrotinib plus capecitabine. Median progression-free survival improved from 8.3 to 30.6 months (HR 0.22, p<0.0001), with a distinct but manageable safety profile characterised by higher haematologic toxicity and low rates of interstitial lung disease.
β These findings support trastuzumab rezetecan as a promising new treatment option.

SERENA-6: Treating Molecular Progression Before It Becomes Radiologic Progression
ctDNA can detect emerging ESR1 mutations months before imaging shows disease progression.
79% of ESR1 mutations were identified before radiologic progression, creating a valuable therapeutic window.
Switching from an aromatase inhibitor to camizestrant, while continuing the same CDK4/6 inhibitor, significantly prolonged progression-free survival.
Demonstrates the power of ctDNA-guided, biomarker-driven treatment adaptation instead of waiting for radiologic progression.
A potential paradigm shift toward earlier, precision intervention in ER+/HER2β advanced breast cancer.
